These are 60+ free HTML form templates (contact, quote, booking, newsletter, RSVP and job application forms) you can copy into any website. Each is plain HTML with a hidden honeypot field that posts to splitforms, so submissions reach your inbox with no PHP or server. Open a template to copy its code, then add your free access key to start receiving submissions.
Every template posts to the splitforms endpoint, so submissions arrive by email and in your dashboard with spam filtering; paid plans can also route signed webhooks. No backend code is required on your side. All 60 templates are organised below by what the form is for: lead capture, bookings, marketing, feedback, support, events, and HR. Many are tuned for specific industries — real estate, plumbing, salons, dentists, photographers, contractors, lawyers, and restaurants.
Each template is free to use; the only optional cost is the splitforms backend that receives submissions, free for your first 500 submissions (a one-time allowance). The sections below walk through setup, field customization, and email or webhook delivery, step by step. Want the fields and examples for one specific business? Browse form templates by industry.

Every template is plain HTML — there's no framework lock-in. Rename a field by changing its name attribute, add a new field with another <input>, mark inputs as required, or replace a select with radio buttons. The splitforms endpoint accepts whatever fields you submit and includes them in the email notification.
For styling, the unstyled markup picks up your site's CSS, so you can drop the form into your existing design system, or browse our styled variants (modern, brutalist, glassmorphism, dark mode, and more). Tailwind, Bootstrap, and shadcn/ui all work out of the box.
Every submission emails the notification address set on that form, on every plan including Free. Add a hidden subject input to control the email subject line. Add a hidden replyto input pointed at the visitor's email so hitting Reply in your inbox goes straight to them. On paid plans (Pro at $5/mo and Annual) you can add CC and BCC recipients so the whole team is looped in.
Need more than email? On paid plans, set a webhook URL on your dashboard to forward every submission to Slack, Discord, Zapier, n8n, your CRM, or your own backend in real time.
Yes. Every HTML form template on this page is free to copy, paste, and deploy, and no template is gated behind a paywall. The splitforms backend that receives the submissions is free for your first 500 submissions total (a one-time allowance). After that, Pro is $5/mo for 5,000 submissions per month with all paid features, and Annual is $49/year for 15,000 submissions per month.
Yes. The templates are plain HTML — rename fields, add inputs, change labels, restyle with your own CSS, switch the layout to multi-column, or wrap them in your framework's component. As long as the form posts to the splitforms endpoint with your access key, submissions arrive.
Yes. The markup is plain HTML with no framework dependency, so you can add Tailwind utility classes or Bootstrap classes such as form-control and btn directly to the inputs. Keep the name attributes, the hidden access_key and the botcheck honeypot as they are.
No. The copy-paste code is unstyled HTML, so the form picks up your site's existing CSS. Want a designed look? Browse our styled variants (modern, brutalist, glassmorphism, dark mode and more), or add your own classes.
Yes. Every field has a label, and inputs use the right types (email, tel), so phones show the matching keyboard. The markup sets no fixed widths, so it fits any screen; give inputs width: 100% in your CSS for a clean single-column layout on phones. When you restyle them, keep the labels and visible focus outlines.
Yes. They use built-in HTML5 validation (required fields and type=email), so the browser checks fields before sending, and splitforms checks every submission server-side for spam. Add minlength, pattern attributes or setCustomValidity messages for stricter rules.
Every submission triggers an email to the notification address on that form, on every plan including Free. On paid plans (Pro at $5/mo and Annual) you can add CC/BCC recipients, forward submissions to Slack or Discord with a webhook, and send the visitor an autoresponder. The dashboard shows every submission in real time.
The submission posts to splitforms.com, where it is spam-filtered, stored in your dashboard, and emailed to you. Nothing runs on your server. On paid plans you can also export submissions as CSV and fire a webhook to your own backend. You can delete the data at any time — you own it.
No. The templates are pure HTML and post to an HTTPS endpoint. They work on static sites (Next.js, Astro, Hugo, Eleventy), no-code builders (Webflow, Carrd, Framer), GitHub Pages, S3, or any shared host. There is no SDK to install and no server code to write.
Yes. Paste the HTML into a Custom HTML or Embed block. WordPress users can also use the splitforms plugin. See our WordPress, Wix and Webflow guides.
Add enctype="multipart/form-data" to the form tag and an input type="file". Storing uploaded files needs a paid plan (up to 5 files of 5 MB each per submission); see the HTML file input guide.
Every template includes a hidden honeypot field that catches automated bots without showing a CAPTCHA to real visitors. splitforms also scores every submission for spam automatically, so junk is filtered before it hits your inbox.
Yes. Paste the same template on as many pages and sites as you like. Each form has its own access key, so create one form per site when you want separate notification emails and submission lists. Every plan, including Free, lets you lock a form to your allowed domains so other sites can't post to it.
500 submissions total, free to start — no credit card required. Need more? Pro is $5/mo for 5,000/month, or Annual is $49/year for 15,000/month.
