Booking / Appointment · React
Time-slot booking with service selection — for salons, advisors, doctors. Free for 500 submissions total — no backend, no SDK, no plugin.

Overview
A booking form feels like the one React feature that has to have a backend — a route to accept the request, somewhere to keep it. It doesn't. Point a client component's <form action> at https://splitforms.com/api/submit, add a hidden access_key input, and the browser POSTs each request — name, service, preferred_date, time_slot — straight into your SplitForms dashboard. No /api/bookings route handler, no 'use server' action, and it still runs on a static export or GitHub Pages where server actions don't even exist. Reach for useState only when you want a live date/time picker; a plain form with named inputs already captures every booking.
Copy-paste code
Replace YOUR_ACCESS_KEY with your splitforms key, paste into a React page, and ship. No build-time integration required.
'use client';
import { useEffect, useState, type FormEvent } from 'react';
export default function BookingForm() {
const [status, setStatus] = useState<'idle' | 'sending' | 'sent' | 'error'>('idle');
// splitforms tracker (drop-off analytics + partial leads), loaded once. It
// finds the form by data-sf-key and adds a hidden sf_sid field to it.
useEffect(() => {
const src = 'https://splitforms.com/embed.js';
if (document.querySelector(`script[src="${src}"]`)) return;

script.src = src;
script.async = true;
document.body.appendChild(script);
}, []);
async function onSubmit(e: FormEvent<HTMLFormElement>) {
e.preventDefault();

// Capture the form now: React clears e.currentTarget once the handler
// yields at the first await, so calling e.currentTarget.reset() later throws.
const form = e.currentTarget;
const data = new FormData(form);
data.set('access_key', 'YOUR_ACCESS_KEY');
data.set('subject', 'New booking request');

}React + Booking / Appointment
React ships nothing for form delivery — it's a view layer. The historical options are an Express server you stand up just for POST /api/contact, a Vercel serverless function with SMTP wiring, or a third-party library like Formik that still leaves the backend problem unsolved. The booking / appointment on this page uses useState for submit state and a plain fetch POST to splitforms — no form library, no context provider, no wrapper component. It works in Create React App, Vite, Remix, and every React bundler because it's just standard FormData and the Fetch API.

Booking / Appointment fields
The posted payload contains full name, email, phone, service, preferred date, notes. Required fields are full name, email, phone, service and preferred date. Rename, remove, or add your own — splitforms accepts any name you POST.

React production notes
React itself ships nothing for form submission — it's a view layer. The historical baseline is one of: an Express/Hono/Fastify server you stand up just for POST /api/contact, a Function-as-a-Service (Vercel/Netlify/Cloudflare) that ends up needing the same SMTP wiring, or a third-party form library (React Hook Form, Formik) that handles validation but still leaves you to operate the backend. Vite, CRA, and Remix all default to assuming you have somewhere to POST — they just don't tell you where. Splitforms is the where: a single fetch call, no library install, no useEffect gymnastics, no Express boilerplate.

Setup
Three actions stand between you and your first booking / appointment submission — no backend, no database, no CAPTCHA library.
Sign in at splitforms.com — your access key is created instantly. No credit card, no setup wizard, no SDK to install.
Drop the form snippet into a React page, component, or layout. Replace YOUR_ACCESS_KEY with the key from your dashboard. The form action is a hard-coded URL — no env vars or build-time wiring needed.
Dashboard updates live on Free. Free includes owner email delivery; paid plans add signed webhooks, CSV export, Slack/Discord forwarding, and BCC to your team.
How SplitForms works
splitforms receives the submission, filters spam, saves it to your dashboard and emails you — then, on paid plans, sends it on to your webhooks and integrations. No backend infrastructure to build.

Point your form to your unique SplitForms endpoint. That's it.

We instantly capture and organize every submission in your inbox.

Send data to email, spreadsheets, CRMs, webhooks, and 7,000+ apps.

No credit card required. Set up in under 60 seconds.
Connect & automate
On paid plans, submissions can also go to Slack, Discord, Google Sheets, Notion, Airtable, Zapier or any signed webhook — and splitforms works on the platforms you build on, from WordPress and Shopify to Next.js. With Stripe connected, your forms can take payments too.

Common questions
No. The booking form is a client component whose action points at https://splitforms.com/api/submit with a hidden <input name="access_key">. The browser does the POST, so there's no route handler and no 'use server' action to write — and it works on a fully static React/Next export, where server actions aren't available at all. SplitForms stores each request and shows it in the dashboard; you export the lot to CSV when you need it.
A picker's internal value never reaches the POST body on its own. Keep react-datepicker as the UI, then mirror the chosen value into a hidden input: <input type="hidden" name="preferred_date" value={date} />. SplitForms reads every named field, so preferred_date becomes a real column in the dashboard and CSV export. A controlled <select name="time_slot"> lands the same way — the name attribute is what matters, not whether the field is controlled.
Yes. Intercept onSubmit, call e.preventDefault(), fetch() the same endpoint with a FormData built from the form, then flip a useState flag to swap in your confirmation component — no reload, and the requester never leaves the page. If you don't need that, drop the JS and let the plain form POST natively. Either path, the paid auto-responder can email the requester an instant 'we got your request' confirmation.
No — it's a form backend, not a scheduling engine, so it records the booking request but never reads a calendar or blocks a taken slot. The workable pattern: wire a webhook (paid) to Slack or Discord so your team sees each request the moment it lands and confirms the slot by hand. Bot spam is filtered by a hidden honeypot field plus a rule-based content filter — no CAPTCHA standing between a real customer and their booking.
Yes. The form is plain HTML with a single POST endpoint, so it runs on any React site without server-side code, plugins, or SDKs. Drop the snippet into a React page or component and submissions land in your splitforms dashboard.

Yes. The template ships with sensible defaults (full name, email, phone, service…) — add, remove, or rename any of them. Splitforms accepts whatever fields you POST.
