EOB basics

What is an EOB?

An explanation of benefits (EOB) is a statement your health plan sends after it processes a claim for care you received. It lists what the provider charged, what your plan allowed and paid, and what you may owe.

An EOB is not a bill

Your provider sends the bill. Before you pay, compare it with the EOB for the same date of service. The amount due should match the patient responsibility on the EOB; if it doesn't, ask the provider or your plan why.

Terms you'll see on an EOB

Amount billed
What the provider charged before insurance.
Allowed amount
The most your plan pays for a covered service, often a negotiated rate.
Deductible
What you pay for covered care before your plan starts to pay.
Copay
A fixed amount you pay for a covered service, such as $20 for a visit.
Coinsurance
Your percentage share of the allowed amount, such as 20%, usually after the deductible.
Patient responsibility
What the EOB says you may owe the provider.
Diagnosis and service codes
ICD-10-CM codes describe why you were seen. HCPCS and CPT codes describe what was done.

How it works

Every path starts with a deliberate action: paste text, choose a PDF, or select a line on a webpage.

01

Choose the text

Paste a line from an EOB, open a text-based PDF in the popup, or select text from a webpage.

02

Read the explanation

KeepMD explains supported codes and insurance terms in plain English, right on your device.

03

Continue only if you choose

Copy the explanation, download a JSON draft, or in Chrome, send an encrypted handoff to KeepMD mobile.

Current features

What the extension does today

Paste to decode

Paste an EOB or medical bill line and get a readable explanation directly in the popup.

Local PDF upload

Choose a text-based PDF. The extension extracts text locally with pdf.js and does not transmit the file.

Selected-text decode

Highlight a line on a webpage, right-click, and review the result in an overlay.

ICD-10-CM and HCPCS

Diagnosis codes, HCPCS Level II service and supply codes, and common EOB terms are explained in plain English.

CPT codes flagged

KeepMD points out possible CPT procedure codes but doesn't describe them, because the official AMA descriptions require a license.

Visible controls

The local activity log is optional, off by default, and can be cleared from the popup.

Decode result

Every code and term, explained in one place.

Paste a line from an EOB or bill, and the popup breaks it down for you.

  • The diagnosis and service codes it found
  • A plain-English meaning for each code and insurance term
  • What to check next, like billed versus allowed amounts

Optional handoff

Encrypted mobile handoff, only when you ask for it

If you choose to continue in KeepMD mobile, the extension creates an encrypted envelope. The server receives ciphertext only; the key stays in the link or QR code fragment.

  • Created only after a user action.
  • AES-GCM encryption happens in the browser.
  • The key is not sent to servers by browsers.

Available in the Chrome extension. The Edge, Firefox, and Safari versions keep everything local and don't include handoff.

Privacy

Promises aligned to the product

  • Pasted, selected, and PDF-extracted text stays on the device for local decode.
  • The local activity log counts only feature taps if you turn it on.
  • The extension does not collect browsing history or scan pages in the background.
Privacy policy

Limitations

Important limitations

  • Informational only, not medical, legal, insurance, or billing advice.
  • Does not determine whether a bill is correct.
  • Scanned or image-only PDFs have no text layer to read, and OCR isn't included.
  • CPT procedure codes are flagged but not described, because official AMA descriptions require a license.

Support and troubleshooting

Where's the KeepMD icon after I install it?

Chrome, Edge, and Firefox put new extensions in the puzzle-piece Extensions menu. Open it and pin KeepMD EOB Decoder to your toolbar.

How do I turn it on in Safari?

Open the KeepMD EOB Decoder app on your Mac once, then turn on the extension in Safari Settings, under Extensions.

Right-click decode isn't showing up.

Highlight the text first, then right-click it and choose Decode with KeepMD. The option only appears when text is selected. On Firefox for Android, paste text into the popup instead.

My PDF shows no text.

KeepMD reads PDFs that contain real text. Scanned or photographed PDFs are images without a text layer, and this version doesn't include OCR.

Why isn't my CPT code explained?

Official CPT descriptions are licensed by the American Medical Association. KeepMD flags CPT-style codes so you know they're there, but doesn't show the licensed wording.

Explore KeepMD

Keep going after you decode the bill.

Decoding a line is often the first step. To check the details behind a claim, request your billing and medical records, learn how to request medical records from a hospital, and keep each EOB next to the rest of your history in the KeepMD health records app.

Ready to decode

Understand the line before the call.

Built for patients, caregivers, and household health organizers who want clearer context before calling an insurer, provider, or billing office.
