Fix presence bugs
Changed the structure and content of the readme.md file.
Fix formatting issue in `.pubnub.yml` for proper change log layout.
Add `BREAKING CHANGE` to `.pubnub.yml` and `CHANGELOG.md` files for useRandomIVs default change.
