o Blank Identification Numbers
o Incorrect patient names, Diagnosis
dates
o Incorrect Dates of Birth
Duplicate Records:
o Duplicate records occur when multiple entries are
created for the same patient
o Evident mostly in IPMS where some clients have
multiple accounts

Documentation in multiple sources e.g.


manual records vs EMR
o Consulting using a physical file vs
documenting on PIMS/IPMS for the same
client/patient
Issuing out a new physical file to the
same patient
o Patient ends up having incomplete data in
one file or no medical history in another
