WORK PERMIT SYSTEM
• The permit indicates that the people undertaking the job , have reviewed
the permit, understood and accept the stipulations mentioned therein.
Combines in one document the following essential things :
• A brief but thorough definition of the work to be done
• Required protective clothing and / or safety equipment
• Control of work execution within agreed time limit

Permit To Work
Definition:
• Performing Authority (PA)
• Persons authorised to carry out the work
• Section Supervisor (SS)
• Person in charge supervises the Performing Authority
• Issuing Authority (IS)
• The departmental manager or his authorized deputy having
proprietary responsibility
• Standby / Attendant
• Person to watch the permitted work being performed
• Rescue Team
• Team of three or more personnel specially trained to carry out rescue 7
Permit To Work
Issuing authority of work permit
• Personnel who are authorized to sign on the Work Permit shall have thorough knowledge
of the work to be performed and the conditions under which the work will be performed.
• Issuing authority is responsible for preparation and handing over of the equipment in
safe condition to perform the work.
• Authority to issue all types of work permits is generally granted to shift
supervisor(PNE/SPNE, UTE,ELE) level of OFFICERS and above.
• The person who has overall responsibility for specifying that control measures to be
implemented in the Permit to Work
• Understand and assess the potential hazards associated with the work.
• Competent to specify appropriate measures to control hazards associated with the
situation requiring the PTW.
• Register all the permits & maintain facility’s PTW Register.
Permit To Work
Executing authority of work permit
• Request for a work permit by executing authority. He shall indicate types of tools and
equipment necessary for performing the work.
• He must:
a) ensure his tools, equipment and accessories are in good condition and all his workers are
provided with proper PPE.
b) He shall understand the hazards involved and arrange for all necessary tests and
inspections
c) Confirm PTW compliance
d) Be able to recognize signs / symptoms
e) Maintain communication with other personnel
f) Be ready to exit, if necessary
g) Follow all PTW requirements.

RESPONSIBILITIES
SECTION SUPERVISOR / DISCIPLINE SUPERVISOR
The person in charge of the potentially hazardous situation and supervises the
Performing Authority.
He must:
Be aware of all the associated hazards
Review the PTW with the Performing Authority
Identify control measures
Ensure all tests and equipment are arranged.
Provide contact with emergency / rescue services.
Prevent unauthorized access
Monitor all PTW requirements.

RESPONSIBILITIES
STANDBY / ATTENDANT
The person (or persons) assigned by the Superintendent to watch the
permitted work being performed.
Fully understand all of the permit requirements
Be aware of the hazards involved
Monitor permit compliance
Maintain a written log / prevent unauthorized entry
Hold details of available rescue / emergency services
Stop / cancel all work if safety is jeopardized
Remain on duty till work finished or is replaced.

RESPONSIBILITIES
NOMINATED PERSON :
Shall be assigned by Pre-Commissioning Engineer/Lead Hook-up engineer to control
and file all the document for pre- commissioning/commissioning
Control all the LOTO hardware i.e. issuance, safe keeping and custody of
tag out/lock out hardware's.
Maintain and keep a register of all electrical safety document
Maintain copies of “closed “ PTW/LOTOs on file for the duration of the
project
Filing and displaying of currently active permits.
Liaise with the Discipline pre-commissioning /hook-up engineer ,issuing
authority and HSE department.

RESPONSIBILITIES
AREA HSE OFFICER :
Is representative of HSE department who shall render technical assistance on HSE
matter, performs safety tests and monitor compliance to PTW/LOTO system
Is representative of HSE department who shall receive and register the issued
PTW in the system.
Keep trail of the currently active permit on the tracking board.

VALIDITY OF THE PERMIT-TO-WORK
• PTW to Specify
• Start and finish dates and times
• Maximum duration-seven days
• Revalidation is required at specified intervals (every shift)
• Suspend work if revalidation is delayed

• Performing Authority, Section Supervisor and Issuing Authority
Signs the close out section after completion of work

ROTATIONAL CREW CHANGES
• Issuing Authority, Section Supervisor, and Performing Authority
• Review all valid PTW with new crew personnel
CHANGE OF WORKSITE CONDITIONS
• If work conditions change, stop work and evacuate area if
necessary
• Only restart work after getting clearance from Section Supervisor
and Issuing Authority

VIOLATIONS OF THE PERMIT-TO-WORK PROCEDURE
Violation of the procedure could be subject to disciplinary action up to and including
termination.
EMERGENCY SITUATION
• Alarm Sounds or emergency arises
• Stop Work
• Copies of PTW are returned to the Issuing Authority
• Start work after revalidating Permit

GUIDELINES FOR ISOLATIONS REQUIRED BY A PTW
• Details of system(s) to be isolated must be listed on the PTW
• An Authorized Person - performs all isolation as per permit
requirements prior to posting PTW at the work site
• Authorized Fitters, Mechanics, Oilers, and qualified Electricians shall
perform mechanical and electrical isolations as required by the PTW.
• All valid permits affected by isolation shall be reviewed to confirm
safety to continue the permitted work.
• All Isolation points should be recorded on Isolation Registration
Sheet (IRS) prior to issuing PTW
• All Isolated systems must be reinstated to their normal operating
condition prior to closing PTW

RESPONSIBILITIES
RESCUE TEAM:
A team made up of a minimum of 3 or more persons trained and available as
indicated on the Permit To Work to initiate and carry out a rescue of personnel within
the permitted area (who are unable to affect a self rescue).
Training must be given to:

• Issuing Authorities
• Rescue Team
• Others, Gas testers, (third party providers)

Toolbox Talk
RESPONSIBILITIES
TOOLBOX TALK
• Toolbox talks are a useful tool in the maintenance of a viable safety and health
education program in the construction industry.
• Schedule TBT before starting of any activity and never skip a meeting
• Limit topic discussion to about 10 to 15 minutes
• Review the talk in advance and deliver the information in your own words
• Encourage group participation.
• Receive and act upon questions, ideas and suggestions
Toolbox Talk
RESPONSIBILITIES
TOOLBOX TALK
• Avoid meetings in noisy areas and try to conduct TBT in the work area to show the
hazards and prevention
• Devote topics exclusively to HSE not any other topic
• Use visual aids where available or appropriate
• Document the TBT with a synopsis and signature of attendees
• If the talk involves an incident, discuss it with injury factors, causes and recurrence
prevention rather than the individual involved in the accident
• If the talk involves work being planned, discuss potential hazards, safety
equipment to be used and basic procedures to be followed
