 Mask like Face
 Tremors and rigidity
 Stooped Posture
 Bradykinesia
 Bradyphrenia
 Salivation
Akathisia

Presentation
 Foot stamping when seated
 Crossing/ Uncrossing Legs
 Rocking Movement
 Unable to sit at one place
Tardive Dyskinesia

Presentation:
 Lip Smacking
 Tongue Protrusion (Fly catching)
 Choreiform hand movement (pill rolling or piano
playing)
 Pelvic thrusting
Treatment of
Acute Dystonia & Pseudo
Parkinsonism
1) Oral Procyclidine

2) I/M Procyclidine

3) Benzodiazepine

Long term Plan

Reduce antipsychotic or change Antipsychotic


Cardiovascular Side effect

 Tachycardia
 Postural Hypotension
 QT Prolongation
 Ventricular Arrhythmia
 Sudden Cardiac Death
Metabolic Side effects

MONITERING
 Blood Sugar
 Lipid Profile
 Weight
 BMI
Neuroleptic Malignant Syndrome
Differential Diagnosis

 Infections( Meningitis, Encephalitis,


Brain Abscess, Sepsis)
 Endocrine(Thyrotoxicosis)
 Environmental (Heat Stroke)
 Toxin (Anticholinergic delirium,
Serotonin syndrome, Substance abuse,
Withdrawal of dopamine agonist
 NMS is a MEDICAL EMERGENCY

 Send CPK & CBC Immediately

 Supportive Care

 Hold ANTIPSYCHOTIC

 Oral Bromocriptine or i/v dantrolene


Take Home Message

 Side effects of antipsychotics esp EPS are dose


related.

 Side Effects of antipsychotics are manageable.

 If side effects are managed properly then patients


with anti psychotics can lead a healthy and normal
life as other individuals.
