Causes of System Failure

The document discusses various causes of system failure at different stages of development and implementation. Pre-implementation factors include lack of research, risk management, and user buy-in. During implementation, poor communication and lack of short-term goals can lead to failure. Post-implementation, declaring victory too soon, failing to learn from mistakes, inadequate user training, and poor organizational fit may cause failures. Failures are classified as issues with development practices, end user problems, or implementation/hardware errors. Remedial measures include management support, user training, communication, testing policies, and backups.

CAUSES OF SYSTEM FAILURE

WHAT IS SYSTEM FAILURE

An information that doesnt perform as expected. Is not operational at a specified time that cannot be used the way it was intended usually accompanied by multiply, parallel manual procedures.

PRE IMPLENENTATION FACTORS

Lack of research, risk management and long term commitment. Information system implementation are getting more complex and therefore it is no longer sufficient to assume that one can competently manage the project without prior research, formal training or necessary expertise. Acceptability of failures If Information system fails the management teams and vendor seems to only melt away in the background and re-group again for the next project.

lack of user buy-in and ownership In addition for making a change users must also be convinced that new system would lead to better work practices. Inappropriate use of technology Technology is a tool and nothing more. Information system cannot exist and solve problem on its own, nor can it increase productivity or profit without the right work process

Poor Development practices Due to wrong assumptions that may be made on the part of the software developers or indeed may be made on the part of the entity requiring the software system.

DURING IMPLEMENTATION
Poor Communication Due to lack of proper communication about the system causes failure because of improper information. lack of short term wins The needs of Information system to be planned achievable short term goals built into the process every short period

POST IMPLEMENTATION FAILURE

Declaring victory too soon The maximum benefit of a system doesnt manifest itself completely immediately after the implementation. Also there tends to be more need for technical support in initial stages. Failure to learn from previous mistakes With each information system, there will be a lesson to be learnt. Mistakes need to be analyzed and the experiences need to be shared. faulty hardware due to the faulty hardware there can be system failure..

Inadequate user training If a user is improperly trained then the likelihood them making serious errors is increased due to lack of knowledge of the system. Poor fit between systems and organisation A poor fit between the system and the organization can lead to various problems. A poor fit between a system and an organization occurs when either the software developers or the entity requesting the software solution, do not grasp the full spectrum of tasks the new system will need to deal with.

CLASSIFICATION OF FAILURES

1.

POOR DEVELOPMENT PRACTICES.

Poor testing of program. Making incorrect assumptions as to the requirements of the system. Providing poor/no documentation. Poor user interface

2 .END USER OR ENTITY PROBLEMS.

Providing incorrect specification for the system. Entering erroneous data. Not providing training for the end user.

3 .IMPLEMENTATION OR HARDWARE
ERRORS
Damaged hardware. Badly designed hardware. A poor fit between the system and the organisation.

REMEDIAL MEASURES TO MINIMIZE

FAILURES
1.

2.
3.

4. 5.

Management support and commitment Adequate training to end users Proper communication between user and the development team Stringent testing policies Maintaining back ups for critical systems
