DISCHARGE

PLANNING
MEDICATION:
Advise the patient to take medications exactly as prescribed:

• Antidiarrheal used for treating of diarrhea of gastroenteritis

• Educate the patient and the family about the importance of adherence
to the medications
EXERCISE/ ENVIRONMENT:
• Bed rest for the patient to decrease the pain and for the recovering of
the patient

• Maintain clean environment and food processing


TREATMENT:
• Educate the mother’s patient to perform Oral rehydration therapy (ORT) .It involves drinking
water with glucose and salts to return the amount of fluids, sugars and electrolytes in the
body to normal levels. Give 10-20 mL every 5-10 minutes of ORS a day until patient get well
• Encourage the mother to provide Nutritonal Therapy. Provide food that contains high fibers ,
bananas, apple sauce and etc. That can help to firm the stool. Avoid food such as: soda,
candy, juices and chocolate milk
• Perform Antimicrobial therapy which is giving antibiotics if prescribed.
• Perform Antidiarrheal drugs such as diphenoxylate, loperamide, tincture of opium, and
paregoric as prescribed. It can reduce symptoms of abdominal cramps, but do not
substantially alter diarrheal stool losses and might even prolong the illnes
HEALTH TEACHING:
Provide patient and relative written and verbal information regarding the following:
• Washing hands with running water and using soap: Germs and viruses can survive up to 2
hours on the surface of the skin, tables, door handles, toys, and so on. Hand hygiene that is
not properly maintained can cause diseases such as diarrhea, cough, runny nose, and fever.
• Consuming nutritious foods: It is pivotal to advice children to be careful in eating snacks
and food/drinks.
• Using clean water and clean latrines for daily needs: It is important to teach children to
defecate in the toilet. Encourage children to use clean water for bathing twice a day,
shampooing every other day, brushing their teeth at least twice a day, cleaning their ears
every time they shower, and washing their feet after each trip and each time going up to
bed.
• Maintaining environmental cleanliness
OUTPATIENT/OBSERVATION:
• Follow up checkup after a week with her pediatrician
DIET:
• The patient should be given several small meals throughout the day
instead of 3 days
• Encourage the patient family to eat nutritious food such as fruits and
vegetables
• Parents should serve children balanced nutritious foods, including
staple foods (rice, noodles, and rice noodles), side dishes (meat, fish,
chicken, tofu, and tempe), vegetables, and fruit every day.
SPIRITUAL:
• Provide spiritual and emotional support
• Encourage the patient family to pray daily and ask God’s continuous
healing
