4. Inventory of Available modules and LAS per week, of Q1 must be


submitted until Sept 6 to prepare additional copies of SLM and LAS
needed on Sept 13

5. Teachers Teaching Online Class must Prepare WHLP ( Activity Plan) to


be uploaded in google classroom and shared to Learners and Parents

6.Prepare Google Classroom, Chat group for Learners and Parents per
Class Per section.

7. All subject teachers are also advise to create group chat of parents
their learners per section under online modality and group text for
modular modality.

8. All subject Teachers must verify who will be the learning facilitator
or their partners in teaching learners at home ( specailly those who are
under modular modality)

grade level head and start including them in your GC and Google
classroom

10. Know you teaching load from your depatment heads and stard
preparing your WHLP, Activity Plan ( for online class), SLM and LAS

11. Same number of LAS, PT, SA per quarter


a least 4 LAS, 4 PT and 2 SA per quarter, alternate schedule,
Collborative PT is required.

12. Have a record of Formative assessment. From the results of


activities in the SLM.

13. Enroll in the Learning Management System of Deped for the access
of Learning materials, video lesson and other IM.

14. Prepare 40 items pretest ( 10 items ffrom competencies per


quarter ) to be admisistered and distributed on Sept 13. Submi until
Wed for reproduction ( students under Modular Modality)

15. Alaways prepare HOTS, localized and contextualized test, PT, video
lessons and other learning materials.

17. Know the procedure of ditribution of printed and digital learning


materials.

18. Learners must submit 1 lexpanded plastic envelop to be used as


learning kit. same color per grade level. Reused previous plastic
envelop. Each earner need 2 learning kit. Distribute the old plastic
envelop. Start naming the envelop. Students under online modality must
have at least 1 learning kit to place important materials for them like
txtbook, card, leaflet and others.

19. Teachers may request learners ( modular) to provide 2 big notebook


per subject to write their answers in the Activities, reflection and
learning output.
