A. Physical

Physiologic Measurements

Temperature: 35.9 C
Pulse: 90beats/min
Respiratory rate:24breaths

• Head- Upright and still, symmetrical facial feature:firm
hard and round.
• Hair- black color fairly distributed, length at shoulder
level.
• Eyes- symmetrical in shape conjugate eye movement;
Chinese eyes, iris appear flat and the same size and
corneas are clear w/o lesions.
• Nose- mucosa free from lesion, symmetrical in shape. No
nasal discharge and flaring.
• Mouth and throat- Lips w/o lesions, oral mucosa and
gingival are pink and moist tongue w/o lesions,
oropharynx and uvula w/o inflammations, tonsil is pink
w/o hypertrophy.
• Ears- symmetrical, no lesions, drainage or
modules, the top of the ear is line up with
the outer of the eye.
• Neck- symmetrical and skin intact no scars. No
visible pulsations, masses, swelling, venous
distention or thyroid gland or lymph node.
• Chest- chest wall symmetrical in shape, no
masses or scars.
• Lungs-no labored breathing, no nasal flaring, no
abdominal sounds in breathing.
• Heart- the pulse is regulate in rhythm when
palpated, no vascular sounds over the carotid
arteries upon auscultation.
