Medical history: diabetic, htn, IHD

Presentation:
11/10/201: came with fever, vomiting, abdominal cramping and
diarrheoa.

Isolates and Iv fluid stared.. bloor tests requested .. PCR
requested.

Hb normal, platelet normal, Weight cell 14000 and PCR showed
norovirus
Electrolytes level abnormality- viral gastroenteritis due to
norovirus.
The virus non-enveloped, with icosahedral geometries. The larger
capsids (38–40 nm) exhibit T=3 symmetry and are composed of
180 VP1 proteins. Small capsids (23 nm) show T=1 symmetry,
and are composed of 60 VP1 proteins.
The virus contain a linear, non-segmented, positive-
sense RNA genome of approximately 7.5 kbp, encoding a large
polyprotein which is cleaved into six smaller non-structural
proteins (NS1/2 to NS7) by the viral 3C-like protease (NS6), a
major structural protein (VP1) of about 58~60 kDa and a
minor capsid protein.
Plan : patient needs to be kept in isolation till electrolytes level
become normal and contiue IV Fluids

- Warn the manager at age care that this virus is highly
competitive contagious.
- Symptoms of this virus... Vomiting fever abdominal cramping
diarrhea. Norovirus infection can cause the sudden onset of severe
vomiting and diarrhea. The virus is highly contagious and commonly
spread through food or water that is contaminated during
preparation or contaminated surfaces. You can also be infected
through close contact with an infected person.
- Diarrhea, abdominal pain and vomiting typically begin 12 to 48 hours
after exposure. Norovirus symptoms last 1-2three days, older adults
and people with underlying disease — vomiting and diarrhea severely
dehydrating ..
- Norovirus infection occurring frequently in closed and crowded
environments> hospitals, nursing homes, aging care centers,
- Isolate any resident with this symptoms
- This virus very dangerous for elderly
