I, MARIA LOREN P. SISON, of legal age, Filipino, single, and a resident of Laoag City,
Ilocos Norte, after having been duly sworn to in accordance with law, do hereby depose and
state:

That I am presently employed as Administrative Assistant II under the General Services


Office (GSO) of the Provincial Government of Ilocos Norte;

That as part of my responsibilities in 2005, I processed Purchase Order No. 0628


intended for the procurement of medicines;

That the said document was originally dated April 21, 2005, and I personally handled its
preparation and initial routing;

That sometime after the public bidding held on May 18, 2005, I was instructed to
retrieve and update the PO;

That upon checking, I observed that the PO was altered to reflect the date May 20, 2005,
instead of the original April 21, 2005;

That I was not given any formal written order explaining the alteration, and upon
inquiry, was told that it was to align the PO with the date after the bidding;

That I found such modification unusual and potentially misleading, and for
documentation purposes, I retained a scanned copy of the original version;

That I am executing this affidavit to attest to the truth of the above events, which I
personally witnessed and handled as part of my work;

That by virtue of a decision promulgated on August 16, 2017 by the Honorable Supreme
Court in G.R. Nos. 221857 and 222020, we were directed to effect the arrest of convicted
public officials Jesus O. Typoco Jr. and Noel D. Reyes;

That on or about September __, 2017, at the last known address of the subjects in Laoag
City, Ilocos Norte, we implemented the lawful arrest based on the warrant issued by the
Sandiganbayan;

That upon approach and confirmation of their identities, we informed them of the
decision and the nature of their conviction and advised them of their constitutional rights;

That both subjects cooperated peacefully, and no untoward incident occurred during
the operation;

That they were brought to the Sandiganbayan Holding Center in Quezon City for proper
booking and documentation;

That we are executing this joint affidavit to formally document and affirm the legality of
the arrest we conducted as part of our official functions;
