Post Ankle Surgery Physiotherapy Protocol

Phase 1: Immediate Post-operative Phase (Week 0-2)

- Goals:

- Control pain and swelling

- Protect surgical repair

- Maintain range of motion in non-affected joints

- Interventions:

- Elevation and cryotherapy

- Non-weight-bearing with crutches or walker

- Gentle active range of motion (AROM) for toes and knee

- Isometric exercises (quad sets, glute sets)

- Ankle pumps

Phase 2: Early Recovery Phase (Week 2-6)

- Goals:

- Gradually restore ankle range of motion

- Begin weight-bearing as tolerated (WBAT)

- Initiate strengthening exercises

- Interventions:

- Partial to full weight-bearing in boot or brace

- Gentle AROM for ankle dorsiflexion, plantarflexion, inversion, and eversion

- Resistance band exercises (light resistance)

- Proprioceptive training (balance board, single leg stance)


Phase 3: Intermediate Phase (Week 6-12)

- Goals:

- Improve ankle strength and endurance

- Enhance proprioception and balance

- Progress functional mobility

- Interventions:

- Full weight-bearing without assistive device

- Progressive resistance exercises

- Gait training and stair climbing

- Low-impact cardiovascular exercises (stationary bike, swimming)

- Continued proprioceptive and balance training

Phase 4: Advanced Strengthening and Return to Activity (Week 12+)

- Restore full strength and function

- Return to work/sports activities

- Interventions:

- Sport-specific drills and functional training

- Plyometrics (if appropriate)

- Agility and coordination training

- Maintenance strengthening and flexibility exercises
