Subject: Reply to Show Cause Notice: Inspection by Quality Reassurance Cell (QRC) on
12.04.2025

Sir,

This has reference to your Show Cause Notice issued vide letter Ref. No.
TDO/QRC/185809/APRIL-25/01 dated 12.04.2025. We would like to give our reply to the
queries raised as follows:

A. HSD tank not in Automation:


REPLY: ATG probe not available in the HSD tank for more than 8-9 months now. We
have filed complaints every month and requested the Automation Engineer to fix the
same. However the same has not been rectified till date. In fact we are suffering due to
this issue as we have to keep applying for manual mode. Also, this has led to frequent
interlocks which often takes time to open. Request to kindly resolve the issue at the
earliest.

B. Measurement could not be checked for XP, as all XP nozzles are looked in Automation
System due to technical problem
REPLY: At the time of QRC inspection there was TT receipt interlock activated for all
XP95 nozzles. Complaint was already raised and TT receipt interlock disabling request
was also sent. However, as the same takes a lengthy procedure for approval the same
was not opened yet at the time of inspection.

In this regard we would also like to further state that such intermittent TT receipt
interlocks or ATG interlocks on a regular basis is hampering our sales. It can also be
said that it is one of the main reasons for customers not sticking to us as most of the
time sales stopped due to this issue.

C. K Factor could not be matched as K Factor are not mentioned in calibration certificate
REPLY: The W&M inspector who came for the last calibration and stamping was a
newly assigned for the task and was not yet aware how to arrive at the K Factor. Hence
the same was not mentioned in his certificate. However, we have requested him to
incorporate the same and it is already a part of the calibration certificate now. He also
gave us assurance that K-Factor will always be mentioned in the certificate henceforth.
D. FO inspection file not maintained for last five years:
REPLY: FO inspection file maintained and updated. However, on the date of QRC
inspection the main manager of the RO was on leave as he had to attend to his nephew
who was hospitalized in Guwahati. The substitute manager who was at the RO that day
was not aware of the cupboard where the said FI inspection file was stored and hence
could not produce it to the QRC inspection team.

Sir, as reasoned above the points mentioned in the Show Cause Notice are not purely
our fault. Hence request you to kindly pardon us on the matters and not initiate actions
against the RO, Ms Faith Services as per provision of clause 8.3 (Major Irregularities)
and clause 8.4 (Minor Irregularities) of MDG 2004.
