AFFIDAVIT
I, KISHORE BURA, aged about 35 years, S/O Gabga Bura, by religion:
Hindu, resident of Village- SOnaipam, P.O.: Lokra, P.S.: Chariduar,
Mouza: Chariduar, District - Sonitpur, Assam, do hereby solemnly affirm
and declare on oath as under;
1. That I am a permanent resident of the above noted place and a citizen
of India by birth. This is true and correct.
2. That my actual and correct name is KISHORE BURA which is correctly
written and recorded in all my relevant documents such as AADHAAR
Card Vide no. 4196 7250 0390, PAN Card DQHPB0758N. This is true
and correct.
3. That, in the birth certificate of my son namely BIMAL BURA vide
registration no. 508/2021 issued by Registrar Births and Deaths
Bhaloukpong, West Kameng Ditrict, APS wherein my name is written
and recorded as SHRI KISHORE BURA; Here the word SHRI before my
name is a mere salutation it is neither my name nor my surname. This
is true and correct.
4. That I hereby declare both KISHORE BURA and SHRI KISHORE BURA
are the names of one and same person having single identity i.e, mine.
This is true and correct.
5. That this affidavit shall be used in support of my above declarations and
it shall be submitted before the authority concerned as and when
required.
And I swear and sign this affidavit on this the 8th day of April,
