BENEFITS COVERAGE
IN-PATIENT BENEFITS

Annual Benefit Limit (ABL) P280,000/year

In-Patient Hospitalization Benefit/year (on top of Philhealth)


Accredited Service Provider/s:
- Non Emergency and/or Emergency treatment Max. of P70,000.00/illness per year or confinement due to
of illness and/or injury accident (deductible from ABL)

Non-accredited Service Provider/s: Reimbursement (80% of the total clinic/hospital charges,


- Emergency treatment of illness and/or injury not to exceed the plan allocation/yr )
Room Type Semi-private
Prescriptive Medicine Reimbursement P1,000.00 per covered confinement
Hospital Income Benefit P300.00/day; max. of 11 days per confinement

OUT-PATIENT BENEFITS
Accredited Service Provider/s: Non-emergency P 6,000.00/quarter (Deductible from ABL)

AVAILMENTS in any IWC-owned/partner clinics are FREE and UNLIMITED which shall not be deducted from the
total ABL of the plan.

• Complete Blood Count (CBC) • Blood Typing
• Blood Urea Nitrogen (BUN) • Cholesterol

• Fecalysis • Fasting Blood Sugar (FBS)
• Blood Uric Acid (BUA) • High Density Lipoprotein (HDL)
3. Diagnostic Procedures (upon order of the in-house Physician) as follows:
• Chest X-ray • Electrocardiogram (ECG)

ANNUAL PHYSICAL EXAMINATION (APE)


A plan member who has been a member in good standing for a minimum of 6 months can avail of APE at any
IWC owned/partner clinics, to include the following procedures:
• Physical Check-up • Chest X-Ray • Fecalysis • Urinalysis • Complete Blood Count (CBC)

FINANCIAL ASSISTANCE
• Natural Death Benefit: P 40,000.00 • Accidental Death, Dismemberment and Disablement: P 40,000.00
• Unprovoked Murder & Assault: P 40,000.00 • Terminal Illness Benefit: P 20,000.00

• Confined in areas and places where IWC WELLNESS AND PREVENTIVE CONSULTANCY, INC. has accredited
