1. One Photocopy of CNIC / Form. B for Compliance.
2. Photocopy of Matriculation / O’ Level Certificate & Mark sheet.
3. Photocopy Intermediate / A’ Level Equivalence Certificate & Mark sheet.
4. Photocopy Migration Certificate those students out of Karachi board education.
5. THREE passport-size recent photographs. Color printouts, mobile-taken images, and filter
photographs are not acceptable. The institute’s Head/Principal signature and stamp are mandatory on the
back of one photograph and the front of two photographs (Don't apply stapler pins on the face).
6. Enrollment fee Rs. 1500/- (JSMU constitutional institute student attached fee voucher) (Signature of the Applicant)

7. Any other information /document can be asked to submit in additional to the above.
IMPORTANT INSTRUCTIONS & REGULATIONS
1. If a candidate remains un-enrolled at the end of the above period, his name can be removed from the college/institution. He shall not be
reenrolled unless he produces the necessary certificate and fee required for enrollment.
2. No application for University Examination or other purpose shall be received from an un-enrolled student unless the enrollment number
is quoted in the application.
