The undersigned is the registered owner of VMAC VENDING MERCHANDISE &


SERVICES located at Apt. 64 Kilbourne Drive, VSU Campus, Baybay City Leyte. I have
applied for the registration of this business last February 2, 2003 with the Bureau of Internal
Revenue with Certificate of Registration OCN No. 2RC0001047021 .

I would like to request for the cessation of the business registration effective June 1,
2019 because my business is no longer existing. The rise of the pandemic during those times I
was not able to inform your office immediately because of travel restrictions and with the lack of
financial resources to sustain the operation of the said business. The business has no
outstanding liability and obligation.

Your favorable action on this request is highly appreciated. Thank you.
