AFFIDAVIT OF LOSS

I, JOSE MANALO, of legal age, Filipino, and a resident of 123


Sampaloc, Manila, after being duly sworn according to law, hereby depose
and say that:

1. I am the person named in the records of the Local Civil Registrar of


Manila, as having been born on February 20, 1952 at Tondo, Manila;

2. I am the child of Juan Manalo and Josephine Manalo;

3. I have been trying to secure a certified true copy of my birth certificate


from the Local Civil Registry of Manila but despite my efforts, I was
informed that no copy of my birth certificate could be found in their
files;

4. Despite diligent inquiries and efforts made with the Local Civil Registry,
it appears that my birth certificate has been lost;

5. As proof of my birth, I am submitting the following documents:

a. A Certification from [Elementary School] attesting to my


enrollment and date of birth;

b. A Certificate of Baptism issued by Santo Niño De Tondo Parish,


which contains my birth date and other relevant details;

6. I am executing this affidavit to attest to the foregoing facts and to


facilitate the process of obtaining a copy of my certificate of live birth.
