VASAI, DIST. - PALGHAR

I, Mrs. RAKESH RAJENDRAPRASAD KANOJIYA Aadhar No.


550820710763, Age 41 years, an Indian Inhabitant, Residing at
Room No. A/301, Sunshine Commercial Complex, Building
Number 3, Near Railway Station, Achole, Nallsopra East do
hereby solemnly state on oath as under :

That Presently I am residing at the above address, with my


family.
I stat that My Mother Named Shobha Rajendraprasad Kanojiya
died on 30/5/2024 & My father Named Rajendraprasad Ratilal
Kanojiya already expired on 24/3/2018.

I state that the deceased have 2 sons named Mr. Rakesh


Rajendraprasad Kanojiya & Mr. Deepak Rajendraprasad Kanojiya
and a daughter named Reena Vijay Kanojiya and we are only
legal heirs of deceased late Mr. Rajendraprasad Ratilal Kanojiya
& Mrs. Shobha Rajendraprasad Kanojiya.

I Stats that my mother has ration card and the said ration card
was in her custody but after her death the same is not tracable,
& it seems that the same has been lost / misplaced. I tried to
search every possible places, but not found.

While I am applying for legal Heirship Certificate before Civil


Judge (JD/SD) at Vasai and unable to submit original Ration Card
Copy hence I hereby execute this Affidavit.

Therefore I am making this affidavit in support of Heirship


application to enable deponents to obtain Heirship Certificate
from the court of Civil Judge, at Vasai.

Whatever stated hereinabove are true and correct to the best of


my knowledge and belief, and if any content found untrue, I
shall be punished u/s. 236/237 of BNS.
