POLYGRAPH TEST REPORT

I. PREPARATORY REMARKS

This report is prepared at the request of Marvin Labe, to whom it is furnished


herewith. Marvin Labe agrees that reports and related information received are strictly
confidential and intended solely for its private and exclusive use or of the said
investigation of the designated agents. Any other use, communication, publication, or
reproduction of this report or any portion thereof, without the expressed written consent
of the said authorities is strictly prohibited. Marvin Labe agrees to indemnify and hold
harmless the examiner against any damage or claims resulting from such unauthorized
use.

II. SUBJECT

Name : Catalino Baluro Jr.


Address : Habitat, Barangay Poblacion, Pamplona Negros Oriental
Position : Utility Worker- LGU Job Order

III. PURPOSE

This test is administered to determine if Mr. Catalino Balauro Jr. is involved in


the alleged theft of the DSWD Office where a certain amount of money, five thousand
pesos only (P 5,000.00), has been left out in the area where he is designated.

IV. PROCEDURE

The term polygraph literally means many writings. The name refers to the
manner in which selected physiological activities are simultaneously recorded. A
polygraph instrument will collect physiological data from at least three systems in the
human body.

The examination included a period referred to as a pre-test, a chart collection, a


chart collection phase and a test data analysis phase. In the pre-test, the polygraph
examiner completed the required paperwork and talked with the examinee about the
test. During this period, the examiner discussed the questions to be asked and
familiarized the examinee with the testing procedure. During the chart collection phase,
the examiner administered and collected a number of polygraph charts. Four tests
were administered to the subject.

Following this, the examiner analyzed the charts and rendered an opinion as to
the truthfulness of the person taking the test.

V. PRE-TEST ADMISSION

Subject stated that he was on duty at the DSWD office in Barangay Poblacion,
Pamplona near the gymnasium last Thursday, October 12, 2023, at around 2-3 o’clock
in the afternoon. It was also indicated in the logbook that he was the only person being
on duty in the afternoon on October 12, 2023. He performed the usual tasks in the
office. Subject also stated that he did not notice nor touched any of the things placed in
the table. He was aware that there is a cctv footage in the office but he said that there
was no electricity that time.

VII. FINDINGS

The numerical scores for the subject test shows, no deception indicated on the
relevant questions.

The polygraph charts were also subjected to a computerized scoring algorithm


called probability analysis. Result of probability analysis indicated a truthful probability.

VIII. POST-TEST ADMISSION

No relevant admission/narration.

IX. REMARKS

The report shall be disposed after one year from date of examination.
