SUBJECT : Request for Polygraph Examination

1. Request conducts polygraph examination on the person of CATALINO


BALAURO JR. in relation to the incident that has been transpired last Thursday,
October 12, 2023.

a) NATURE OF CASE : THEFT

b) VICTIM : MARVIN REBUTAZO y LABE

c) SUSPECT : CATALINO BALAURO JR

d) TDPO : At about 2-3 o’clock in the afternoon of


OCTOBER 12, 2023 an alleged theft incident transpired in in the office of the DSWD in
Barangay Poblacion, Pamplona, Negros Oriental wherein victim MARVIN LABE, 25 yrs old,
single, utility workder and resides at Bobon street, Pamplona Negros Oriental was according
to his statement being assumingly robbed by the above-mentioned person.

The victim reported the incident the day after he lost his money in the office and the
Municipal Police Station responded represented by Patrol Man Lance Barba responded by
checking the vicinity and getting details in the office. He also tried to get a footage of the
date being mentioned on the cctv but it has been stated that the line of electricity has been
tripped off.

The MPS is still in an ongoing investigation.

2. Further request that this Office be furnished a copy of examination


result for our reference.
