GREETINGS!

Pursuant to the provisions of Rule 21, Section 1C, Para 4, Sub-para b and c
and Section 1B, Para 1, Sub-para b of the NAPOLCOM MC No. 2016-002, otherwise
known as the “Revised Rules of Procedure Before the Administrative Disciplinary
Authorities and The Internal Affairs Service of the Philippine National Police,” dated
March 9, 2016, you are hereby notified that an administrative complaint for LESS
GRAVE NEGLECT OF DUTY; para d, fail to comply with any lawful order or instruction
of a superior officer or the Chief of Police, has been filed against you by PCOL JOEL T
ADA, ACS, PCADG.

WHEREFORE, you are hereby directed to submit your


counter-affidavit/comment under oath through mail, courier or in person within five days
from receipt of the complaint. You may attach in your answer/counter affidavit pertinent
documents in support thereof and submit the same before the Office of the Chief
Investigator/Pre-Charge Evaluation, Police Community Affairs and Development Group
building at Camp Crame, Quezon City. Failure to comply within the required period will
constitute a waiver on your part to present evidence and the investigation of this case
will proceed ex-parte.

Attached is a copy of the complaint of PCOL JOEL T ADA, ACS, PCADG. and
its annexes for your perusal.
