· Strictly use SABISTON, SCHWARTZ OR Harrison’s Principles of Internal Medicine 20 th edition, Bate’s Guide to Physical Examination and History Taking, MIMS, or Clinical Practice Guidelines as your refer-
ence in answering this case.
· Indicate the reference and page number in the column provided. Failure to write the reference will incur deduction from the total grade.
TASK # 1:

Make a script as to how you will endorse this patient to your


consultant simulating a clerk-consultant interaction. This script
includes the following:

1) who you are;

2) patient identification information;

3) concise history of the problem;

4) supporting diagnostic tests data

5) any suggestions for further evaluation;

6) any dilemmas, and;

7) disposition of the patient

Make a script that can be reported in less than 2-3 minutes.

Provide a diagram of the pathophysiology of this case of what happened to this patient. The diagram should contain the following:
· Predisposing and precipitating risk factors
· Signs and symptoms
· Physical examination findings
· Diagnostic test results
· Medications and their target areas in the pathophysiology
