AIIMS NORCET - Myocardial Infarction (MI) Notes
Definition
Myocardial Infarction (MI) is the death of heart muscle due to lack of oxygen caused by reduced
blood supply (ischemia). It is commonly known as a heart attack.
Causes
- Atherosclerosis (plaque buildup in coronary arteries)
- Thrombus (blood clot blocking coronary artery)
- Coronary artery spasm (sudden narrowing of arteries)
Signs & Symptoms
- Severe chest pain (radiates to left arm, jaw, back)
- Sweating, nausea, vomiting
- Shortness of breath
- Palpitations & dizziness
- Silent MI (common in diabetics)
Diagnosis
- ECG: ST-elevation (STEMI) or ST-depression (NSTEMI)
- Cardiac Markers: Troponin I/T (Gold Standard), CK-MB
- Echocardiography: To check heart function
Treatment (MONA-B)
- **M**orphine: Pain relief
- **O**xygen: Improve oxygen supply
- **N**itrates: Vasodilation
- **A**spirin: Prevent further clot formation
- **B**eta-blockers: Reduce heart workload
Complications
- Arrhythmias (irregular heartbeat)
- Heart failure
- Cardiogenic shock
- Pericarditis (inflammation of heart lining)
Prevention
- Healthy diet (low-fat, low-salt)
- Regular exercise
- Control of diabetes & hypertension
- Smoking cessation
