AFFIDAVIT OF CONSENT

I, TARCILA P. ROTA, of legal, Filipino, married and resident of 18 Damortis St.,


Damar Village, Quezon City, herein represented by my attorney-in-fact Jhon Vincent C. Rontale
per Special Power of Attorney executed January 13, 2024 before notary public Atty. Artemio CI
Vendivil and registered in his notarial register as Doc. No. 381; Page No. 78; Book No. 135;
Series of 2024, after having been duly sworn to accordance with law, by these presents, do
hereby declare and manifest:

That I am the administrator of Lot Not. 1-B with an area of 235 square meters situated in
Brgy. Estancia, Kalibo, Aklan covered by Tax Declaration No. 17-01-009-01475 including the
residential/commercial building built thereon;

That I hereby signify my written consent, freely and voluntarily, to allow my niece Jhona
Coching Cuales to use a portion of the ground floor of the building found on the above-described
parcel of land to conduct business, the Theo Sari-Sari Store.

That I am executing this Affidavit to attest to the truth of the foregoing facts as to my
undertaking to give consent to my niece Jhona C. Cuales in support her application for business
permit.
