my legal heirs would face in the event of my death with out nomination registered in your Bank records.
FEMA Declaration
1. I/We hereby declare that the transaction, the details of which are specifically mentioned in the
Schedule hereunder and the opening of a demat account-NRI (Repatriable/ Non Repatriable) / inward
/outward remittance does not involve, and is not designed for the purpose of any contravention of the
provisions of the aforesaid Act or of any rule, regulation, notification, direction or order made there
under from time to time.
2. I/We have complied and shall continue to comply with the aforesaid Act and the
rules/regulations/directions thereunder.
3. I/We also hereby agree and undertake to give such information/documents as will reasonably satisfy
you about the accounts/transaction in terms of above declaration.
4. I/We also understand that if we refuse to comply with any such requirement or make only
unsatisfactory compliance therewith, the Bank shall refuse in writing to undertake the transaction and
shall if it has reasons to believe that any contravention/evasion is contemplated by me and report the
matter to Reserve Bank of India. I/We further declare that the undersigned has the authority to give this
declaration and undertaking on behalf of the company.
WhatsApp Consent
1. Customer authorizes the Bank official of Kotak Mahindra Bank Ltd (KMBL) to enter KYC and other
necessary details on his/her/their behalf in the savings account opening form as per the information and
instructions given by the Customer for the AADHAAR BASED biometric savings account opening.
Customer confirms that the details filled are documented by Bank official in his/her presence and the
same have been reviewed and verified by Customer in the application form.
2. Customer hereby agree/s and give voluntary consent that the name in the account will be the name as
per his/her AADHAAR Card.
3. Customer depositing funds using the funding facility available in the App should ensure that the
remittance is made from Customer’s own/ close relative’s bank account and not from a Third Party
Bank account. Bank shall be at liberty to refuse to open the accounts funded from a Third Party Bank
account at its sole discretion.
4. Customer agrees to provide the accurate documentation and information as listed in the app for the
purpose of account opening. Customer/s understand/s and agree/s that failure to provide requisite

documentation and information shall result in rejection of application by KMBL. The customer agrees
that KMBL has every right to close the account of the customer , if there is any erroneous, false,
incomplete or misleading information provided by the customer or for any other reason whatsoever or
if KYC documents submitted do not comply with the KYC norms of KMBL, without assigning any
reason.
5. Customer agrees that the Bank reserves the right to hold the account/s in Debit Freeze or close the
same even after generation of account number or account activation/ completing the Full KYC process
in case of any discrepancy found as part of regular monitoring and document verification activities.
6. This account opening process is also not available for customers who are FATCA reportable. Such
customers are requested to approach the branch and to comply with the requirement for opening an
account.
7. Customer declares and confirms that the Bank shall be entitled to rely on all/any communication,
information and details provided on the physical form and all such communications, information and
details shall be final and legally binding on the Customer.
8. Customer understands and confirms that the Bank has every right to cancel/ reject the account opening
application, if the details provided by him/her/them are found to be inaccurate, incorrect or false by the
Bank or for any other reason whatsoever without assigning any reason thereof. In such an event the
bank shall not be liable to pay any interest on the amount of deposit & the refund of amount deposited
in the account will be refunded to the source account. In such an event, bank will retain the documents
/ Photographs and any other signed document submitted and customer shall return the instant account
opening kit (if any) to Bank.
9. Customer understands and acknowledges that the account number generated will be in debit freeze
status to avoid any unauthorized transactions and debit transactions will be allowed after completion of
account opening formalities and activation of account. This application is available for opening a
Savings and Current account by Resident Indian Individuals only.
10. Customer understands and agrees in case the account is not activated within 30 working days from the
date of acknowledgement of account opening application being submitted, full amount will be credited
back to the customer via the same channel of funding which might take 6-8 additional working days.
11. Customer understands and agrees that interest on the amount funded will be paid subject to activation
of the account and on availability of clear funds in the account (not applicable to Current Account).
12. The customer authorizes KMBL to share his/her mobile number and address with the respective
vendor to verify customer’s address at any point of time.
13. Customer declares and confirms that the shared information is true and correct to best of his/her/their
knowledge. Customer hereby agrees and confirms that all such acts and deeds done for and on
Customer behalf by the executive who has visited him/her/them for account opening formalities shall
be binding on the Customer and on Customer’s successors & legal heirs.
14. I understand that I will be bound by and have to abide by the Bank’s general terms and conditions
available on the bank’s website and savings bank account rules. I www.kotak.com understand that the
Bank reserves its right to revise its service charges as laid down in the General Features Schedule and

15. For Kotak Mahindra Bank Ltd employees (Staff Account): I understand that I need to update the
'Salary Account number; in Kotak Worklife App of the Bank. Post successful updation of the account
numbers in Kotak Worklife App portal by me, the salary account will be converted into Staff account
under same CRN, if applicable. In case account numbers are not updated in Kotak Worklife App, my
salary will be on hold, the account will continue to remain as a regular salary savings account and
General Schedule of Features and Charges for Salary Account will be applicable.

I/ We understand and agree that Payment Gateway will be activated by default for the customers
registered for Net Banking with unconditional transaction rights. I/ We understand and agree that Net
Banking, Phone Banking access, if applied for, is applicable to all Deposit accounts, existing or to be
opened in future. I/ We understand and agree that existing / future accounts will be linked by default to
Debit Card/ Payment Gateway and for delinking Debit Card/ Payment Gateway from the account I/We
have to visit the nearest branch to place the request. I/ We agree to be bound by the said Terms and
Conditions including those excluding/ limiting the Bank liability. I/ We understand that the Bank may
at its absolute discretion, suspend any of the Services completely or partially without any notice to
me/us. I/ We agree that the Bank may debit my/ our account for service charges as applicable from
time to time. I/ We hereby agree, declare and confirm that the funds routed by me/ us in my/ our
account will be from my/ our own source of funds acquired legitimately and such funds are acquired
not in contravention of any applicable laws or rules in force, enacted / issued by Statutory or
Regulatory Authority from time to time or funds so routed have not been or acquired by way of
evasion of government levies/ taxes. I/ We shall advise the Bank immediately in the manner as agreed
by me/ us and acceptable to the Bank, in case of any change in the above details an information given
by me/ us. I/ We do hereby authorize the Bank to conduct my/our credit history verification with
CIBIL or any other Credit Rating Agency. I/We understand that bank may at its absolute discretion
reject my/our request to open an account without assigning any reason.
2. The opening of the Account is subject to verification, and Bank reserves the right to reject the request
of the Applicant to open an Account without providing any reason. In the event where the documents
submitted by the Applicants or Customers for opening an Account and/ or availing the Bank’s products
and services or for processing the transactions thereon are incomplete or discrepant, Bank may at its
discretion contact the Applicant for rectifying such discrepancies and/or for certain additional
verification/ documentation requirements. In case highlighted discrepancies remain unresolved or upon
non-receipt of required documents within the stipulated time period, if any, Bank, shall have the right
to reject the opening the account of the Applicant/ service request / application in accordance with its
policies and procedures and retain the application forms and documents provided for account opening,
including photographs in case of rejection of the said application. In such an event the Bank shall not
be liable to pay interest on the amount of deposit and shall refund the money received, if any, to the
source account/applicant.
3. Activmoney (applicable only if opted for): I/ We have been explained about the features and benefits
of ActivMoney (Auto Sweep-Out, Sweep-In) and I/We have understood and agree for the same.
4. Statement of Account:I/ We am/are aware that the Bank will forward statements by way of email on
monthly basis to my/ our registered email ID. I/ We agree that Bank would be deemed to have
delivered the statement to me/ us, on Bank sending the email containing the statement as an attachment
to me/ our registered email id. In case email has not been provided by me/ us, physical statement at
quarterly intervals would be sent to my/ our mailing address. I/ We am/are also aware that if I/ We
have provided my/our email id but still wish to opt for physical statement at quarterly intervals, I/ We
need to provide a specific request for the same. Any person resident in India collecting and effecting/
remitting payment directly/ indirectly outside India in any form towards overseas foreign exchange
trading through electronically/ Internet trading portals would make himself/ herself/ themselves liable
to proceeded against with for contravention of Foreign Exchange Management Act (FEMA), 1999
besides being liable for violation for regulations relating to Know Your Customer (KYC) Norms/ Anti
Money Laundering (AML) Standards.
5. I/We authorize the bank to: (1) share my transaction and demographic details, with
regulatory/Enforcement Authorities as well as Kotak group companies, whenever needed. (2) update
my details in the Bank’s records based on the documents submitted by me. (3) Send me information
from the Central KYC registry, via SMS or Email, at my registered mobile number or email address.
(4) Update my KYC information with the central KYC registry, as per the directions of government of
India. (5) Open my account using the name registered with UIDAI (Aadhar) or the Income Tax
department (PAN), in place of any name mentioned on this form.
6. SMS Banking & Alert Facility: I understand that (1) signing up for SMS alerts will allow the Bank to
send me account transaction and balance information on my email id and/or mobile number. New
alerts may be added from time to time. (2) The Bank or its Partners may call SMS or email me
information or seek feedback related to Bank’s product and services. (3) I may withdraw my consent at
any time in writing or by registering for Do not Call on the Bank’s website www.kotak.com. (4) some
mandatory alerts or some deemed important by the Bank will be sent even if I have not subscribed for
them. (5) All alerts will be sent to the first holder or Guardian for Individual accounts and to all
Holders for Joint Accounts. (6) Charges for the services will be as mentioned in the General Schedule
of Features. Mobile Banking : I understand that Mobile Banking Services will be activated if I opted
for Netbanking or a Debit card.
7. Customer voluntarily agrees to use his/her Aadhaar details for opening AADHAAR based biometric
savings account. Customer agrees and hereby authorize Bank to fetch their personal details from
UIDAI. Customer authorizes Bank to use his/her Aadhaar details and core biometric information to
authenticate and verify his/her identity.
8. I have updated the Nominee details and I authorize the bank to update the same in their records as
Form DA1, which will be binding on me, including my heirs, legal representatives, successors and
executors. I hereby authorize the bank to send the copy of the said Form DA1 to me through email id
provided by me to the bank for communication purpose and I hereby confirm and agree to that bank
shall not be responsible for any misuse of information sent through email.Account nominee will be
considered as the default nominee for Debit Card and linked Insurance claims.
9. Other Banking Products: Customer agrees and authorizes Bank to share his/her contact details with
the respective bank team/department for communication purpose in case Customer wishes to avail
other banking facilities at the time of account opening.
10. The customer agrees that the email id given by him/her will be updated in the account only post
self-verification by the customer over an email sent by the bank. Until then, no communication or
statement will be sent to the email ID provided by the customer.
11. The customer herewith agrees to be contacted by the bank to receive information in respect of account
maintenance, alerts, payments due, updates on existing and new products, servicing of account for
sales, marketing or servicing their relationship with KMBL, its group companies/associates or agents
through Telephone/Mobile/SMS/Email etc. Further he/she understands that the consent to receive
calls/communications shall be valid and shall prevail over their current or any subsequent registration
of their mobile number for NDNC and shall continue to be treated as customer consent/acceptance.
1. Silk offers Kotak Rewards on Silk Platinum Debit Card on meeting the qualifying criteria. Silk
Programme privileges and rewards are subject to maintenance of requisite average monthly balances in
the savings account. ATM cash withdrawals are not eligible for rewards. Reward credit confirmation
and other Silk Programme related communication are sent to the registered e-mail and/or mobile no. of
Sole/Primary account holder maintained with the Bank and it will be the responsibility of the
customer/account holder to have a valid e-mail id & mobile number updated with the bank at all times,
failing which, customer may not receive the reward credit confirmation mail / SMS and other account
related information and Bank shall not be responsible for such failure on part of customer. Customers
with mobile numbers / e-mail id registered for DNC / NDNC will not receive SMS confirmation from
us unless a specific consent has been provided to the Bank. In the event of non-receipt of reward credit
confirmation within the subsequent month from the last day of qualifying month, customers to contact
their relationship manager/branch/customer service helpline within 50 days from the date of qualifying
for the rewards. Any claims of non-receipt of the rewards beyond the stipulated period shall not be
entertained. The Bank shall not be liable for any loss or damage arising or resulting from delay in
transmission delivery or non-delivery of electronic messages or any mistake, omission, or error in
transmission or delivery thereof or in deciphering the message owing to any reason beyond the control
of the Bank. The Bank also reserves the right with / without notice to terminate or alter this offer and
terms and conditions or modify existing process of redemption / introduce any additional process at
any time without assigning any reason whatsoever. The decision of the Bank with respect to this Offer
shall be final and binding. All disputes arising out of or in connection with this Offer shall be subject to
the jurisdiction of courts at Mumbai. 9. 1. 2. 3. For NMC waiver in Junior Account Minimum monthly
RD or MF-SIP instalment value of Rs. 2000/- (Rs1000 or above for Semi-Urban & Rural Branches);
Minimum tenure of 36 months; Maximum cap on RD monthly instalment: Rs. 50,000 for tenure up to
120 months / Rs. 1,00,000 for tenure up to 36 months
Any person who is 55 years or above and is Indian resident is eligible to avail benefits under Grand Savings
Programme. The benefits/offers under this programme can only be availed by the Primary Account Holder.
1. Kotak Rewards are offered on spends done through your debit card. ATM withdrawals are not
considered for rewards. The rewards qualification confirmation/ credited information and other Grand
Savings Programme related communication are sent to the registered e-mail and/or mobile no. of Sole/
Primary account holder updated with the Bank. Customers with mobile numbers registered for DNC /
NDNC will not receive SMS confirmation from the Bank. The rewards under the Grand Savings
Programme is subject to maintenance of requisite average monthly balance in the account.
2. Home Banking (HB) services will start within a month from the date of A/C opening. The date
selected for HB can be changed with 30 days prior intimation. Once it is changed, in subsequent
months cash delivery by Bank would happen on that date. If on the specified date of HB, due to
holiday/Sunday/ or any other reason, Bank is unable to execute the transaction, the cash shall be
delivered on the next working day. These services are provided on a No Warranty or Risk basis,
therefore the Bank shall have no liability to the customer or any other parties for any indirect,
incidental, punitive or consequential costs, damages or otherwise with respect to the Bank offering
these services and the Bank shall not be liable or responsible on account of any misuse of the services
and the Bank stands indemnified in this regard. The Bank on a best-effort basis will execute the HB
transaction within the specified duration as mentioned above, and shall not be responsible for delay or
non-delivery on the specified date. This facility is provided once a month and in HB active locations.
The charges applicable will be as per standard GSFC and basis the account type. Wherever HB is not
available, the customized cash delivery option will be HBNA by default. The Bank also reserves the
right with / without notice to extend or terminate the above offers or alter the terms and conditions or
modify existing offer or introduce any additional process at any time without assigning any reason
whatsoever. The decision of the Bank with respect to this Offer shall be final and binding. General
Terms and conditions: The terms and conditions related to the Grand Savings Programme will be over
and above the standard terms and conditions of the account and the services which shall be deemed to
be incorporated herein by reference and apply as the context may require. The discount on locker rent
will be applicable for locker allotment in the financial year of enrolling in Grand Savings Programme.
Locker discount is not applicable for customers who convert their existing account or open another
account to enrol into this programme. Locker allotment is subject to availability. The Bank shall not be
liable for any loss or damage arising or resulting from delay in transmission delivery or non-delivery of
mailer/ electronic messages or any mistake, omission, or error in transmission or delivery thereof or in
deciphering the message owing to any reason beyond the control of the Bank. For Privy League and
Wealth customers enrolling in Grand savings programme, respective GSFC for Privy League/ Wealth
savings account shall apply. A Grand priority card that enables you to capture vital health related
information will be sent to you post account opening. In case of non-receipt of the same kindly contact
your home branch. Any replacement of Priority/First aid card issued later shall be charged as per
annual fees of Classic Debit Card mentioned in GSFC
1. Entry to the said programme is by invitation and at the sole discretion of the bank. Meeting the
programme eligibility criteria is not an implicit invitation to the programme. The customer reserves the
right to decline the invitation by submitting a written request at the bank branch or through e-mail at
[email protected].
2. Kotak Mahindra Bank reserves the right to appoint / change the Relationship Manager of the customer.
3. Kotak Mahindra Bank reserves the right to amend, alter, modify, change of vary all or any of the
benefits / services offered and the eligibility criteria to the programme.
4. The benefits of the programme are applicable till the time the customer is a part of the programme.
Kotak Mahindra Bank reserves the right to withdraw the membership to the program without prior
consent from the customer. In the event of withdrawal of programme entitlement service charges
applicable shall be as applicable prior to invitation to the programme.
5. The benefits of the programme are subject to prevailing regulatory guidelines for various customer
types (Resident Individuals, NRIs, Exporters, Importers, Business entities, etc.)
6. Nothing contained herein shall prejudice or affect the terms and conditions accepted by the Customer
at the time of opening the account.
7. Kotak Mahindra Bank shall not be liable for any loss /complaint arisen out of any transaction or
interaction with any alliance partner.
8. The programme benefits may be extended to members under the customer's business and/ or family
relationship at the discretion of the bank. However, this does not imply that the customers bear any
financial liability towards the said members when programme benefits are extended or withdrawn
unless specified otherwise in an agreement between the customers and Kotak Mahindra Bank.
9. The customers' contact details (addresses, contact numbers, e-mail IDs etc.) are to be provided by the
customer and the responsibility to intimate the bank of any change lies with the customer.
10. Privy League customers holding a Global Trade Account (GTA) shall enjoy trade charges as per the
GTA declaration they have signed. Privy League customers holding a GTA must maintain Privy
League Programme eligibility criteria in order to continue enjoying Programme benefits.
DECLARATION: I/ We am/are aware that for premature withdrawal of Deposit(s) [for mode of operations
Either or survivor, Anyone, Former or survivor], bank will allow withdrawal of deposit (premature / on
maturity) as per the operating instruction provided by holder(s) at the time of creation of Deposit or any valid
subsequent request submitted to bank. *I/We hereby authorize the Bank that in event of death of anyone of
the Depositor(s), the bank on receipt of written request from the surviving Depositor(s), as per the mode of
operation, so allow the surviving Depositor(s) to prematurely withdraw the fixed deposit without seeking
concurrence from the legal heirs of the deceased depositor(s), if I/we have given a joint mandate for
premature withdrawal of deposit by the surviving Depositor(s) at the time of booking the deposit or
subsequently. I/We am/are aware that Fixed Deposit booked under 'Premature Withdrawal not allowed’
option cannot be withdrawn till maturity. For Fixed Deposits booked under ‘Premature Withdrawal not
allowed’ option linking of fixed deposits to Saving/Current Accounts and Auto Renewal option is not
available and I/We understand and agree to that the Lien cannot be initiated by me/us and OD facility cannot
be availed on ‘Premature withdrawal not allowed’ Fixed Deposit. I/We further declare and confirm that any
modification to the above authorization/mandate shall be only by way of joint instructions by all the
applicants/Joint holders. I/We am/are aware that penalty charges will be levied for the premature withdrawal
of deposits as per the applicable terms and conditions of the bank and I/we have been informed about the
applicable penal interest rate for premature withdrawal. The calculation of interest is basis 365 days in a
non-leap year and 366 days in a leap (calendar) year. No interest will be payable for NRETDs staying less
than 1 year. ‘Tax Saving term deposit’ Deduction u/s 80C of the Income Tax Act is subject to fulfilment of
condition mentioned in the Income Tax Act, 1961, No premature withdrawal of principal or accrued interest
before completion of 5 years is permitted under Tax Saving term deposit scheme, In case of joint holder type
deposit, deduction u/s 80C shall be available only to the first holder of the term deposit ‘Tax saving term
deposit’ cannot be pledged to secure loan or as security to any other asset. I/We am/are aware that TDS is
also deducted on unpaid interest accrued at the end of a financial year on 31st March TDS will be recovered
from principal of Deposit Amount, when interest earned is insufficient to recover TDS. I/We have read
understood the terms and condition governing the opening of on account with Kotak Mahindra Bank Ltd. and
those relating to various services including term deposit(s). I/We accept agree to be bound by the said
Terms Conditions including those excluding/limiting the bank liability. I/We understand that the bank may
