REQUIREMENTS FOR APPLICANTS


Read and follow the instructions, for strict compliance:
Please bring the following documents below in HARD COPY.
SUBMIT YOUR COMPLETE DOCUMENTS AND COMPILE AS LISTED BELOW.
Note: Incomplete requirements shall not be entertained

1) Photocopy of Vaccination Card

2) Application letter (state the position applying for) addressed to:

Ms. Jean A. Wong, MMPA


Chief, Human Resource Management Division

3) FULLY-ACCOMPLISHED PHC Application Form (see folder and read the instructions carefully)

4) FULLY-ACCOMPLISHED Personal Data Sheet with Passport-size ID picture

5) Work Experience Sheet (download at csc.gov.ph)

6) Photocopy of Transcript of Records (for college graduate)


*Form 137 (for Highschool Graduate)

7) Photocopy of Diploma

8) Photocopy of Certificate of General Weighted Average (to be requested in school)

9) Photocopy of Summary of Related Learning Experience (RLE) - for Staff Nurse Applicants only

10) Photocopy of Authenticated PRC ID

11) Photocopy of Authenticated Board Rating

12) Photocopy of Authenticated Civil Service Eligibility (if applicable)

13) Photocopy of Certificate of Good Moral Character (School / Employer / Baranggay)

14) Photocopy of Certificate of Employment (if applicable)

15) Photocopy of Seminar/Training Certificates attended after graduation (if any)

16). Photocopy of Updated NBI Clearance or copy of Claim Stub (if not yet available)
