Having examined the Tender Document including the Invitation to Bid, Instruction to Bidders,
form of Contract Agreement, Conditions of Contract, Plans, Drawings, Specifications and Form of
Bids including all Addenda, I/we hereby accept the same as conditions in making this Bid.

I/we offer to furnish all Labor, Materials, Equipment that are required to execute, complete
and maintain the entire Contract Works in conformity with the said Tender Documents at the unit
bid price that we present in the Bill of Quantities shown in Annex “A” with the total amount of

Contracts Works within days from our receipt of your Notice to Proceed and I/we shall
