5. Appointment Order (CSC Form No. 33/KSS; for applicant/s with previous employment in the
government);
6. Service Record (For government employees);
7. Original or Authenticated copy of Transcript of Records and Diploma;
8. Original or Authenticated Certificate of Eligibility/Rating/License (with supporting documents if name
appearing on Eligibility Certificate is different from name in the PDS) Proof of Eligibility;
9. Original or Authenticated/Certified copy of Certificate of Trainings/Seminars;
10. Authenticated copy of Certificate of Employment;
11. Authenticated Copy of Individual Performance Commitment and Review (IPCR)/Performance Rating
