Purpose of assessment: To assess the personality of a child

Test administered: Draw-a-person Test (DAP-T)


Test Behaviour: The child was co-operative, could comprehend the instructions, and was
able to communicate.

Interpretation: DAP-T was administered on a female child. The child had first drawn a
figure of the same sex, taking a time of 4minutes, 51seconds and the second drawing was that
of the opposite sex (male) taking a time of 5minutes, 36 seconds. In the first drawing, the
female figure can be seen holding a poster with the slogan “Safe life and animals”, whereas,
no such activity can be seen in the second drawing. The participant stated the girl is aged 10
years, whereas, the boy in the drawing is aged 12 years. After interpreting both the figures, it
seems that the participant views male as inactive, passive, introvert and female as active,
extraverted. She is frequently self -conscious or introverted. She is an individual with a great
deal of drive and ambition. The strokes in the drawing are an indicative of the participant
being self-involved, introverted or anxious. She might also portray severe disturbed
interpersonal relationships as the head was drawn at last. The drawings are also an indicative
of the participant being verbally aggressive. The participant has also expressed rejection and
contempt.
Impression: Therefore, the drawings suggest that the child shows traits of introversion,
anxiety and underlying aggression.

3. Purpose of assessment: To assess the structure of personality of the individual.
4. Test administered: Rorschach Inkblot Test.
5. Test Behaviour: The individual was cooperative, could comprehend the instructions,
and was able to communicate.
6. Interpretation: The individual’s responses suggest a balanced cognitive style, with
the subject demonstrating the ability the ability to integrate structure, creativity, and
emotional insights. A moderate reliance on form and a high focus on standard details
indicate a preference for practicality and conventional thinking, with minimal
attention to unusual elements. The participant exhibits a balanced approach to big-
picture thinking and detail orientation, as well as equilibrium between imaginative
thought and emotional responsiveness. Longer reaction times for chromatic cards
suggest careful emotional processing. The strong focus on whole human and animal
representations reflects concrete and realistic thinking, with limited use of abstract or
unconventional perceptions. Overall, the participant appears grounded, adaptable,
and emotionally stable, with no significant distortions or pathologies evident in their
responses.

1. Chief Complaints:

* Recurrent intrusive thoughts of contamination, leading to significant distress and avoidance


of public spaces for the past 8 months.

* Persistent doubts about locking doors and turning off appliances, occurring multiple times
daily for 1 year.
* Rechecking appliances and doors 10–20 times each evening for 2 years.

* Distress about the inability to control repetitive behaviors, present for over 1 year.

Duration of Illness: 6 months

3. HISTORY OF PRESENT ILLNESS:

The patient reports experiencing intrusive thoughts about contamination, particularly with
germs and dirt, which cause significant distress. These thoughts are accompanied by an
overwhelming fear of getting sick or spreading illness to others. To alleviate these fears, the
patient engages in excessive handwashing and cleaning rituals that often last up to 30 minutes
per session and occur at least 10–12 times a day. He acknowledges that the rituals provide
temporary relief, but the anxiety returns shortly after. The onset of symptoms began gradually
around 3 years ago, following a severe cold and flu episode. Since then, the compulsive
cleaning and avoidance behaviors have worsened, severely interfering with his daily
functioning

The patient has not sought any formal treatment but occasionally takes over-the-counter sleep
aids.

* Major depressive disorder

* Bipolar disorder

* Psychotic symptoms

* Substance abuse
