RESOLUTION NO. 04, s- 2021

“SANGGUNIANG KABATAAN OF BARANGAY ADAMS AUTHORIZED SK


CHAIRPERSON JESTHER A. TARNATE TO REPRESENT THE SK OFFICE OF
BARANGAY ADAMS IN THE PROCESSING OF APPLICATION OF TAX
IDENTIFIACATION NUMBER (TIN) AND OTHER DOCUMENTS.”

WHEREAS, the SK Council of Barangay Adams, an association existing as a


political body of Republic Act 10742 with the principal place of business at Barangay
Adams, Adams, Ilocos Norte.
WHEREAS, in a meeting held on the aforementioned principal place of the business
last DECEMBER 04, 2021, it was agreed unanimously approved by the SK Council that SK
Chairperson JESTHER A. TARNATE be hereby authorized to represent the SK Office of
Baranggay Adams, Adams Ilocos Norte in the processing of the TAX IDENTIFICATION
NUMBER and official documents related to the foregoing;
WHEREAS, the SK Council further agreed that such authority granted to SK
Chairperson JESTHER A. TARNATE include, among others, to sign and execute documents,
to process the application for aforementioned documents;
NOW, THEREFORE, BE IT RESOLVED, AS IT IS RESOLVED BY THE
SANGGUNIANG KABATAAN OF BARANGAY ADAMS to authorize SK Chairperson
JESTHER A. TARNATE as the representative to the BIR for the application of TAX
IDENTIFICATION NUMBER (TIN) and such other documents as may be necessary.
RESOLVED FURTHER, to furnish copies of this resolution to all concerned government
offices/ agencies for their information and appropriate action;
APPROVED this 4th day of DECEMBER 2021 at Barangay Adams, Adams, Ilocos Norte.
