Test Center Addition Request Form


F5 has intentionally limited the number of locations available to take F5 Certified exams to provide
better security over the exams and reduce the amount of exam fraud in our program.

However, you may need to request a new Pearson VUE “Select” testing location if:
• There are NO test sites within 55 miles/100 km of your location
• The testing location is too busy and cannot schedule ANY exams for at least one month (or
more)

To apply, please fill out the form below and click the SEND link at the bottom. We will try to respond
within 30 days of your application.
