BURIAL ASSISTANCE (BODBA) FOR FUND ALLOCATION BY THE DATE OF REQUEST DEPARTMENT OF BUDGET AND MANAGEMENT (DBM) NAME OF CLIENT/ORGANIZATION: Kindly Check Documentary Requirements the (as of August 1, 2024) Documents Needed Basic Requirements 1. Certification that the barangay official was incumbent at the time of his/her death using the prescribed template (issued by CONTACT DETAILS: the DILG C/MLGOO) 2. Certified true copy of the death certificate of the deceased barangay official 3. Photocopy of any valid government-issued ID of the claimant If Claimant is the Widow/Widower: (Certified or True Copy) OTHER NEEDED INFORMATION, if any: 4. Marriage Certificate Relationship to the DBO:
If Claimant is the Parent of the deceased Barangay Official : (Certified or
SIGNATURE: True Copy) 4. Birth Certificate of the deceased barangay official If Claimant is the Son/Daughter or Brother/Sister of the deceased barangay official 4. Birth Certificate of the claimant RECEIVED BY: DATE AND TIME RECEIVED If claimaint is the Common-Law Spouse/Parent of the deceased barangay official: 4. The legal heir shall execute an Affidavit or Special Power of Attorney In the absence of primary and secondary beneficiaries/ claimants: 4. An affidavit of two (2) interested persons or affidavit of self-adjudication shall be executed by the claimant to support that he/she is the only heir of the deceased barangay official If a designated beneficiary/legal heir is a minor : 5. A designated beneficiary/legal heir who is a minor shall be represented by a guardian. An affidavit of Guardianship shall be executed for the purpose In the case of multiple designated beneficiaries/claimants of deceased barangay officials: 5. A Special Power of Attorney, which shall be executed as supporting document, except if the claimant is spouse. Only one check shall be issued to the person whom the beneficiaries agreed from among themselves to receive the check. Other supporting documents from the claimant (whichever is Receiving Officer applicable): ENDORSED TO RESPONSIBLE OFFICER: DATE AND TIME ENDORSED 6. Photocopy of Police Report, in case the matter of death of the deceased barangay official is not a natural cause 7. Affidavit of two (2) disinterested parties shall be executed if there is a discrepancy in the name of the deceased barangay official or claimant 8. For beneficiaries of IPMRs, if applicable, in the absence of the documents mentioned above, affidavit of two (2) disinterested persons shall be required 9.
