Application For Pension for 60 Year Age / Family/ Widow

Pension (Part A)
List of Required Documents:
1. Life Certificate of beneficiary in the month of January Every
Year. Aadhaar based online life certificate through the Jeevan
Pramaan Website www.jeewanpramaan.gov.in which
generates digital life certificate may be submitted.
2. Death Certificate (in case of beneficiary death)- to avail
family/window pension
3. FIR/ Post Mortem Report (in case of accident)
4. Bank Passbook or Cancel Cheque of Applicant
Application For Pension for Permanent Disability (Part B)
List of Required Documents:
1. Disability Certificate indicating the percentage of permanent
disability issued by the Civil Surgeon Office
2. Life Certificate of beneficiary in the month of January Every
Year. Adhaar based online life certificate through the Jeevan
Pramaan Website www.jeewanpramaan.gov.in which
generates digital life certificate may be submitted
