Intership-Contract
The Host Training Establishment (HTE), Company Name, with company address,
represented by Company Representative agrees to accommodate the STUDENT INTERN,
Intern’s Name, who is an Accountancy student of the College of Business, Economics, and
Accountancy, Mariano Marcos State University, for the purpose of providing experiential
learning and training in the Company. This training period will commence on June 13, 2018
and will end on July 24, 2018.
2. MMSU, through its SIPP Coordinators shall have the following duties and responsibilities:
a. Prepare the INTERNSHIP PLAN of each STUDENT INTERN for the whole duration of
the training.
b. Facilitate the screening of students who applied for industry training.
c. Ensure that all requirements (scholastic and documentary) are complied with and in
order.
d. Coordinate with the HTEs prior to deployment of STUDENT INTERNS.
e. Conduct MANDATORY PRE-INTERNSHIP ORIENTATION/SEMINAR prior to
deployment to internship venue including among others work ethics and anti-sexual
harassment laws as pre-requirement.
f. Inspect internship venues and sites to ensure the safety and welfare of the STUDENT
INTERNS.
g. Monitor and assess STUDENT INTERNS periodically (or if the need arises) to obtain
feedback about the performance of the STUDENT INTERNS in their respective
training venues.
h. Consult and assist STUDENT INTERNS in resolving problems/issues encountered.
i. Facilitate the evaluation and validate the result of the internship of students at the end
of the internship period.
j. Issue a final grade to the STUDENT INTERN upon completion of requirements within
the prescribed period in accordance with the University OJT Manual on grading
system and performance assessment of HTEs.
k. Submit to the OJT COORDINATING OFFICE semestral internship report on the
number of student interns per program, their issues and concerns and actions taken to
address the issues among others.
3. The STUDENT INTERN shall discharge the following duties and responsibilities:
a. Satisfy the orientation-seminar prior to deployment.
b. Accomplish Daily Time Record (DTR) duly attested by their respective supervisor/s.
c. Attend his training religiously and satisfy the official time prescribed by the HTE.
d. Perform tasks and activities indicated in the internship plan.
e. Adhere/conform to all rules and regulations prescribed by the HTE while on training.
f. Maintain confidentiality, when and where appropriate, during and after the internship
period of all the data, business or trade secrets where such information is not within
the public domain and is indicated or understood to be confidential.
g. Maintain a daily journal of internship describing the internship activities, any problem/s
encountered, and his or her reflections on the internship experience.
h. Complete the agreed duration of his or her internship (as provided by the CMOs of
each degree-granting program); in case the STUDENT INTERN will be unable to finish
his or her internship within the prescribed period, he or she shall inform the SIPP
Coordinator in writing of his or her intent and reasons to prematurely end his or her
internship, at least three (3) working days before his or her last day of internship.
Failure to complete the internship program without prior notice and valid
cause/reason(s) disqualifies the STUDENT INTERN from retaking the internship
program with the concerned HTE.
i. Prepare and submit terminal report of internship activities and accomplishment report
to the SIPP Coordinator.
4. Under this training agreement/internship contract, the following terms and conditions are
likewise included:
a. The student shall be considered as an intern and not as an employee of the HTE for
the duration of the internship period. Therefore, it is expressly understood that there
will be no employer-employee relationship between Host Training Establishment
(HTE) and Student Interns of MMSU.
b. Both the HTE and the STUDENT INTERN have the right to pre-terminate the on-the-
job training if:
i. the STUDENT INTERN does not show the required interest, maturity, or
discipline during the training period, of there is serious misconduct.
ii. the HTE does not provide the kind of responsible training as agreed upon.
The terminating party will inform the SIPP Coordinator, the Department Chair, The College
Dean, and the OJT Coordinating Office Chief before any decision to terminate is made and
finalized. The aforementioned individuals may intervene in order to rectify the situation in the
interest of all parties concerned.
