Electrical Cable of ACS entangled with temporary Guardrail

SUMMARY OF INCIDENT
Description:
Dated on 08.03.2023 around 11:50 PM an incident occurred at The Crest
Project Site, Tower A. During CW-05 ACS Jumping, main electrical power cable
of CW-05 ACS entangled with the temporary guardrails of staircase shaft – 01.
As the cable was entangled in temporary guardrail and the ACS was moving
upward direction in jumping, the cable got stretched which resulted in damage
to the cable insulation causes ACS electric panel shutdown and ACS jumping
stopped.
No injury or property damaged were reported.
Findings of Investigation:
During the investigation, it was revealed that,
• Involved crew in CW05 ACS jumping failed to assess the risk.
• SIMOPs planned by site team without suitable plan.
• Area inspection conducted by HSSEI and area electrician hours back
before ACS jumping.
Corrective and Preventive Actions taken:
• No signage and barricades provided for awareness near to cable roll.
• No watchman provided to monitor safe unwinding of ACS cable roll. • Cable drum to be used for safe unwinding of cable roll during ACS jumping.
• No suitable engineering control was present for safe handling of ACS • Safe system of work method to be adopted – SIMOPs should be pre planned for safe
