1. Scope Of Work

This method statement covers the requirements of Concrete Pavement on EPC


Pembangunan SP ABG Stage 1 – AKASIA BAGUS.

2. Reference

3. Definitions
COMPANY : PT. Tripatra
CONTRACTOR : PT. Asuka Engineering Indonesia
SUB-CONTRACTOR : The company that manufactures or supplies equipment and
services to perform duties specified by CONTRACTOR.
"VENDOR" covers Manufacturer/ Supplier/Sub-vendor.

4. Responsibilities
Group / person who will be responsible in the site is listed bellow :
1) Project Manager
2) Construction Manager
3) Site Engineer
4) QA/QC Inspector
5) Safety Officer
6) Foremen / Work Supervisors
7) Equipment Operators
8) Skilled and unskilled labourers

4.1. Project Manager


1. Ensuring that the project works in his zone are carried out in accordance with
Company policies and in accordance with the requirements of this Quality
plan.
2. Ensuring the full compliance of subcontractors with Company Quality policies
and with the requirements of this Quality plan.
3. To ensure that all the equipment required to execute the works according to
the construction programme are available, in good condition, and provide any
additional equipment that might be required.

4. To co-ordinate with the Construction Manager, Project Engineer, Safety


Engineer, Foreman and Surveyor for a safe and proper execution of the
works.
5. To guide specific attention to all safety measures in co-ordination with the
safety officer/engineer.

4.2. Construction Manager

1. Ensure area is ready and safe to start the works.


2. Set up required equipment and plant through discussion with the Project
Manager and Project Engineer/Works Supervisor.
3. Ensure the works are carried out according to the specification, quality and
approved shop drawings.
4. Liaise and co-ordinate with the Project Manager for the agreed sequence of
works with respect to the construction programme.
5. Allocation of required manpower through co-ordination with the Project
Manager.
6. Provide the risk assessments for the works in hand.
7. Provide sufficient and safe access for operatives, trucks and pumps.
8. Take precautionary measures with regards to protecting works from hot
weather, wind, rain and sun.

4.3. Site Engineer

1. The engineer shall carry out his duties in a manner that shall be co-ordinated
by the project manager on a daily basis, and shall ensure proper distribution
of the workforce and equipment at required locations.
2. To be aware of test frequencies related to the formation level.
3. To control disposal of waste excavated material according to the instructions
received from the Project Manager.
4. To co-ordinate with the Safety Officer to maintain safe working and proper
housekeeping of the site. To comply with the safety measures and ensure that
all Safety Officer Teams are aware of the same to prevent accident and loss.
5. To monitor and check all activities and ensure that works shall be carried out
according to specifications, quality and approved drawings.
6. To inform the QC Inspector of the areas ready for inspection.

4.4. QA/QC Engineer / Inspector

1. Ensuring that Consultant/Client inspection requests are implemented.


2. Compilation of all necessary quality control checklists
3. Assisting Consultants during the Inspections.
4. Coordinating with the third party lab regarding tests and results.
5. The control of work performance by means of checking the work before
consultants’ inspection and issuing RFIs & punch lists as necessary.
6. Completion of documentation to verify the work performed.
7. Controlling all inspection activities on-site in line with ITPs.
8. Ensuring that all test equipment including surveying equipment is calibrated
and is suitable for use on-site.

4.5. Surveyor

1. To establish benchmarks from agreed reference points, provide required


setting out and level markings and follow up with regular checks.
2. Co-ordinate with the Project Engineer / Foreman and ensure the approved
shop drawings/construction drawings shall be implemented properly.
3. Maintain survey details and reports, periodically check the progressing works
and advise the project manager of any deviation from the drawings.

4.6. HSE Engineer/Officer


6. To ensure implementation of all safety measures related to the nature of
works being carried out, and in accordance with the Project Safety Plan.
7. To ensure that all the persons involved in the works are aware of their
responsibilities, and that they have enough understanding of the safety
procedures.
8. The safety officer in co-ordination with the Project Manager shall ensure that
all the implemented safety measures are effective enough to maintain safe
working on the site.
9. To maintain continuous inspections of the site activities, advise and train
persons on a daily basis to prevent accidents and personnel injury.
10.To give special concern to housekeeping, and ensure that the site is
maintained clean and tidy.
11.To ensure all the relevant safety sign boards for different works are in place.

4.7. Foremen/ Works Supervisor

1. Ensure the works are progressed in the sequence as agreed with the Project
Manager.
2. Liaise with the Project / Construction Manager for the allocation of the work
force, ensuring adequate manpower is available.
3. Liaise with the site manager to ensure all the required plant / materials are
available to construct the works.
4. Full time supervision shall be required to ensure the works are carried out in
accordance to specifications, quality and approved drawings.

5.3. Method Implemented


1) Preparation Work
a. The Project Engineer and Foreman studied the shopdrawing for work
preparation
b. Surveyors measures the work area and nailed the necessary point and area
limit with block of woods (bouwplank) sprayed with marks (elevation mark,
mid-build mark, and sloping mark) for excavation work.
c. Grader Excavator / Bulldozer strip the work site for safety and workability
later on.

2) Compacting Subgrade of Existing / New Limestone


a. After the work area stripped, Vibro Roller came in and compacted the area.
Following by that, the compacted area continuously watered with controlled
water consent.
b. Vibro Roller compacted the area with 6 times passed in 500 m2 areas.
c. After 6 times passed, the compacted area tested with CBR, and if the result
is enough (CBR > 25%), the Vibro Roller will go onto next area that needed
to compact, if its not, the Vibro will need compact the area 6 times passed
more.
d. Surveyors checked the elevation of compacted area. If its enough (same as
planned elevation) then go onto next layer of pavement.

3) Set Out the Formwork and Rebar


a. After the first layer of pavement done. Dump Truck that carried the
Gravelsand dropping the material, along the pathway of pavement.
b. The mound of sand (from dumptruck drop) is graded by Bulldozer.
c. The Bulldozer also helps compact the area by passing the sand that
graded by it.
d. The surveyors will stays in area and checked the elevation, if the layer
of elevation reached 100 mm, Surveyors give signals to Vibro
Operator. Then the Vibro Roller came in.
e. The Vibro Roller Compacted the area by 2 times passing in 500 m2
areas, without trembling the machine.

f. After 2 times passed, the compacted area tested with Plat Bearing,
and if the result is enough, the Vibro Roller will go onto next area that
needed to compact, if its not, the Vibro will need compact the area 2
times passed more.
g. Surveyors checked the checked the elevation of compacted area. If its
enough (same as planned elevation) then the lean concrete is spread.
h. Make a lean concrete from cement, black sand, and coral with 1 : 4 :
5 composition for lean concrete, then pour and spread it on top of the
compacted sand.
i. After the lean concrete has 1 days age, spread the plastic sheets and
then form a formwork and wiremesh form (that already form) on the
site.
j. Make sure the formwork and wiremesh is solid and sturdy
k. Make sure the wiremesh is tied with wire and bottow of wiremesh has
a decking between wiremesh and plastic sheet
l. Make sure the formwork doesn’t make leak the concrete, make sure
its all sealed.
m. After everything set, pour the ready mix due to strength specification
or pour the fresh concrete with approved composition into the form
work. Then use the Vibrator to flow and compact the concrete, and
don’t do it to much because it can lead into segregation. Do it every 30
cm of concrete thickness.
n. After the concrete poured all and stiff, scratch the top concrete with
manual tools.
o. In 12 hours laminate it with wet cloth / tarpaulin for curing.
p. Do the curing every day until 28 hours, it prefent concrete from
cracking.
q. After 7 days, the formwork can be opened

4) Longitudinal Joints
a. Longitudinal Joints is placed in the middle of the pavement.
b. Set out the longitudinal joint, D16 rebar per 750 mm, place it on 10 cm
top of lean concrete with rebal decking.
c. Paint the middle of longitudinal joint rebar with anticorrosion paint
around 10 cm.
d. Make sure the joint is sturdy and solid

e. After the concrete poured and become stiff, the gap between concrete
pavement in longitudinal join is filled by PVC filler and the top of filler
is sealed with Joint Sealant

5) Transverse Construction Joints


a. Transverse Construction Joints is placed in the transverse axis of the
pavement, and it set up in every 6 m.
b. Set out the transverse joint, Ø25 dowel per 300 mm, place it on 10 cm
top of lean concrete with rebar decking. Covered with PVC Pipe Ø33-
300 for the sleeve.
c. Make sure the joint is sturdy and solid
d. After the concrete poured and become stiff, the gap between concrete
pavement in transverse join is filled by PVC filler and the top of filler
is sealed with Joint Sealant

6) Transverse Construction Joints


Transverse Expansion Joints is placed in the intersection between concrete
pavement northeast pathway and northwest pathway . And after the
concrete poured and become stiff, the gap between concrete pavement with
another pavement is filled by PVC filler and the top of filler is sealed with
Joint Sealant.

7) Transverse Construction Joints


Transient Joints is placed in the intersection with another pavement. And
after the concrete poured and become stiff, the gap between concrete
pavement with another pavement is filled by PVC filler and the top of filler
is sealed with Joint Sealant.

8) Isolation Joints
Isolation Expansion Joints is placed in the intersection with drainage. And
after the concrete poured and become stiff, the gap between concrete
pavement with another pavement is filled by PVC filler and the top of filler
is sealed with Joint Sealant.
