Type of Incident The temporary power distribution panel board was found with open
door panel.
Dust and dirt accumulated inside the enclosure with can potentially
cause contact resistance to the electrical connection between wires
and circuit breaker terminals.
Contact resistance is one of the causes of heating at the terminals of
the circuit breaker which could lead to arc flash incident.

Corrective Actions

1. Immediate Closure of Panel Door:


• Ensured the panel door is securely closed and locked to prevent further
accumulation of dust and dirt inside the enclosure.
• Implementing regular checks to confirm that the door remains properly closed
during operations.

2. Cleaning of Panel Interior:
• Shut off the power supply to the panel board.
• Safely clean the interior of the panel, removing all dust, dirt, and debris from the
electrical connections and other components.
• Inspect and clean the circuit breakers, terminals, and wires to ensure proper
contact and connection.

3. Inspection and Testing of Connections:


• After cleaning, inspect the electrical connections for signs of damage, corrosion,
or wear due to accumulated dust and dirt.
• Perform continuity and resistance testing to verify the integrity of the connections
and confirm that contact resistance is within acceptable limits.
• Tighten any loose connections or replace damaged components as necessary.
4. Preventive Measures:
• Installing a dustproof gasket or seal around the door to prevent dust and dirt from
entering the enclosure in the future.
• Schedule periodic inspections and cleaning of the temporary power distribution
panel board to maintain its safety and performance.
• Provide training to personnel on the importance of keeping electrical enclosures
properly sealed and free from contaminants.
5. Documentation:
• Record all corrective actions taken, including cleaning, inspection, and testing
results.
• Document the preventive maintenance plan for future reference.
