AFFIDAVIT OF COMMITMENT

I, Paul Vincent B. Bañados, have read and understood the policies and
guidelines of this institution and voluntarily submit myself to abide by the
school’s and the department’s rules, regulations, and considerations
without any personal reservation or condition.

I commit to extend extra service to support and partake in the school’s


efforts to deliver education and its services for its students.

I also willingly accept my teaching loads (subject assignments) and


subject overloads (subjects exceeding regular loads) and am fully aware of
the implications, which require me to exert more effort in executing my
duties and responsibilities.

Further, I commit to ensuring the quality of my teaching methodology,


delivering outputs as required by my superior, meeting deadlines for
submitting my obligations, and participating in institutional activities.

By affixing my signature, I attest that the above affidavit is true and
