ELECTRONIC HEALTH RECORD

This is an integral part of health care industry. The use of pen on paper by nurses and doctors
to note patients vitals is gradually becoming obsolete, especially in the developed world. EHR is
an electronic version of patients health record which before now was created , used, and stored
in a paper chart. It contains the following patients health information:

Patients demographic data such as age,name, gender, address, occupation etc

Progress note

Medical history

Diagnosis

Medications

Immunization

Vital signs

Laboratory investigations and results

EHR is more than just a computerized version of patients chart in a providers office. It is a
digital record that provides comprehensive health information about the patient .it is desinged
to share information with other health care providers.

DEFINITION OF ELECTRONIC HEALTH RECORD (EHR)

It can be defined as a systematic collection, storage and retrieval of patients health information
in a digital format.

These records can be shared across different health care settings. Records are shared through
network-connected entreprise wide information system or other information networks and
exchanges.

BENEFITS OF EHR

 EHR is used for other reasons aside charting patientsc records. Data from patients
records are used by providers to improve quality outcome through their care
management programmes.

 It can be used to improve the quality of care given to patients by using the data and
analytics to prevent hospitalization.

 EHR systems are designed to store data accurately and to capture the state of the
patient over time.
 It eliminates the need to trackdown previous patient medical records and helps in
ensuring that data are up to date.

 It allows for open communication between patients and health provider while providing
privacy

TYPES OF EHR

 CLOUD-BASED EHR SOFTWARE: the 21st century is often referred to as the age of the
cloud. A cloud based EHR allows you to store and access patients data online. The use
of cloud-based EHR platforms enables clinics to save overhead cost as third party
vendors handle external servers. Patients get unrestricted access to their medical profile
but not allowed to edit them.

 PHYSICIAN-HOSTED SYSTEM: this type of application stores patients data on


physician’s or organization’s server.This can be costly as it is capital intensive for the
purchase of the hardware and the softwares. It can be used in a large organization
where the revenue and income would be able to cover the over head cost for
purchaching and maintaing the programe.

 REMOTELY HOSTED SYSTEM: here, patients data are stored on the vesndors servers
instead of the physicians server. This considerably cuts down the cost of handling
hardware and software and it is a more advanced technology than the physician-hosted
system.
