AFFIDAVIT

I, SUSAN MAHUSAY CUA, of legal age, Filipino, married, with office address
at 4897 Valenzuela Street, V.Mapa Santa Mesa, Manila, after having been duly
sworn to in accordance with law, hereby depose and state that:

1. I am a the registered owner and the sole proprietress of Brotherhood


Plastic Enterprises with address at 4897 Valenzuela Street, V.Mapa Santa, Manila,
Brgy. 600;

2. Last March 23, 2023, I was in the Our Lady of Lourdes Hospital at
around 11:00 AM for my medical follow-up check-up when the Philippine Drug
Enforcement Agency (“PDEA”) conducted a random inspection in my business
address, and thus, I was not present during the said inspection;

3. In the said inspection, the PDEA discovered that some of the IPA and
Toluene chemicals were not stored in one designated area of the factory. This is
because the company decided to order more than its usual supply of because the
supplier informed us that the prices of the same will increase in the following days.

4. The company specifically designated the staircase area of the factory


as the storage of its IPA and Toluene drums for convenience of the machine
operators. However, since the company ordered more than its usual number of
drums, we decided to temporarily use the basement of the plant to store the other
IPA and Toluene drums because the designated storage area is already full. Simply
put, we merely grabbed the opportunity to overstock our usual IPA and Toluene
supplies due to its imminent price increase and thus, we were constrained to
temporarily store some of the said chemicals in the basement of the plant in the
meantime because the designated storage area of the plant is already full when the
PDEA conducted its investigation.

5. Finally, the IPA found by the PDEA during their inspection is also
stored alongside the Toluene drums in the staircase area of the plant as mentioned
above. Since there was an overstock of the same due to the circumstance
mentioned, we also decided to temporarily some IPA drums that no longer fit in the
designated storage area in the plant’s basement.

6. Nevertheless, the undersigned and the company undertakes that this


incident will no longer happen in the future, and that there is no intention of me and
my company to bypass or violate our laws regarding the storage of chemicals.

7. I am executing this affidavit to attest the truth of the foregoing and for
any legal purpose that it may serve.
