Daily pre-requisite form for permit to dump


The pre-requisite form is required to be filled for ‘Loading/Dumping’ job wherever heavy vehicle is involved, filled
in by the concerned TSL and Vendor supervisors at the beginning of every shift and shall be attached with the work
permit issued for loading/dumping activity).
ONE FORM TO BE USED FOR ONE JOB/SITE ONLY.
