TUITION PRESCHOOL CONTRACT 2023-2024

By initialing each item and signing below, I understand and agree to the following terms:

_____ 3 yr. Old ONLY - My preschooler is enrolling for the first time in the 3- year-old program in the Alpine School
District Preschool (not repeating). My preschooler is/will be 3 years old on or before September 1, 2023.

_____ 4 yr. Old ONLY - My preschooler is enrolling for the first time in the 4-year-old program in the Alpine School
District Preschool (not repeating). My preschooler is/will be 4 years old on or before September 1, 2023.

_____ I understand that my preschooler’s monthly tuition payment covers the cost of the teacher, aide, and materials.
Without my monthly tuition payment, ASD would not be able to provide a preschool program. I understand that
the 1st tuition payment is due on the FIRST day of school and the recurring tuition payments, will continue
from October through May on the 1st of each month. A late fee of $15 may be charged if payment is made
after the 10th of each month.

_____ I will enroll for the automatic payment system to pay for my preschooler’s tuition by one of two ways:
E-Check (ACH) OR Credit Card/Debit Card
I understand that the paperwork for either E-Check or Credit Card/Debit Card is required to be completed during
preschool registration. I understand that failure to pay by the end of the month WILL result in the
withdrawal of the child, and I am responsible to pay the remaining unpaid balance.

_____ I understand that if my payment is rejected (for any reason), Alpine School District may at its discretion, attempt
to process the charge again within 15 days. I agree to an additional $35.00 charge for each attempt, which will
be added to the authorized recurring payment. I certify that I am an authorized user of this credit card/bank
account and will not dispute these scheduled transactions with my bank or credit card company, so long as the
transactions correspond to the terms indicated in this authorization form.

_____ I understand that my preschooler’s recurring payments will remain in effect until I CANCEL in WRITING. I
need to notify the Preschool Department 10 days prior to the next billing date by emailing:

_____ I understand that buses are NOT provided for Preschool. I am responsible for transporting my preschooler to and
from preschool. My preschooler will arrive on time and will be picked up on time each day. I will follow the
school’s policies and procedures including its check out/check in and visitor policies.

_____ If my preschooler has any signs of illness–coughing, fever, eye or nasal discharge, vomiting, diarrhea, etc.–I will
keep my child home. If my preschooler is absent, I will call the school and excuse my preschooler.

_____ I understand that regardless of how much my preschooler attends each month, I am expected to pay the total
tuition amount each month.

_____ My preschooler doesn’t have an IEP - Individualized Education Plan (except for Speech).
