CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This Contract is executed this ______________ in Digos City, Davao


del Sur by and between:

1. ____________________ OFFICE, with office at Lapu-lapu


Street corner Jose Abad Santos Street, Digos City,
Davao del Sur, represented by ATTY.
_______________________-, hereinafter referred to as the
FIRST PARTY;

WITNESSETH:

WHEREAS, the FIRST PARTY is a law office who also subsidiarily


engages in the processing of land titles for a fee;

WHEREAS, the SECOND PARTY aims to transfer the title of his


property to his name. This property is more specifically described as
follows:

NOW, THEREFORE, the PARTIES herein agree that the FIRST


PARTY shall process the titling of the said property to the SECOND
PARTY’S name with the following conditions:

1. The FIRST PARTY shall be paid a service fee of TEN


THOUSAND PESOS (P 10,000.00) for such processing,
exclusive of any taxes and fees that shall be imposed by the
respective government agencies concerned for the processing
and granting of such land title;

2. The SECOND PARTY shall give a non-refundable DOWN


PAYMENT of FIVE THOUSAND PESOS (P 5,000.00) upon the
signing of this contract;

3. The SECOND PARTY shall pay the remaining balance in the


amount of FIVE THOUSAND PESOS (P 5,000.00) upon claiming
the Land Title from a duly designated representative of the
FIRST PARTY;

4. The service fee shall not include transportation expenses


going to and from the respective government agencies
concerned, which expenses shall be reimbursed by the

SECOND PARTY upon presentation of receipts by the FIRST
PARTY;

5. The service fee does not also include notarial services;

6. This agreement shall be effective upon the signing if this


document and will remain in effect until one or both parties
signify its desire to cancel this agreement with proper notice
of FIFTEEN (15) days;

7. Any and all legal actions emanating from this Contract shall
be filed in the courts of Digos City, Davao del Sur.

The parties do hereby agree, commit and bind themselves to


comply faithfully and strictly with the terms and conditions of this
Contract.

IN WITNESS WHEREOF, the parties have hereunto set their


hands this __________________ in Digos City, Davao del Sur, Philippines.
