Assessing Body Temperature Using Mercury Thermometer

PURPOSES
 To establish baseline data for subsequent evaluation.
 To identify whether the core temperature is within normal range.
 To determine changes in the core temperature in response to specific therapies (e.g.antipyretic medication,
immunosuppressive therapy) 
 To monitor clients at risk for imbalanced body temperature (e.g. infection, expose to temperature extremes)
EQUIPMENT
 Thermometer & thermometer sheath or cover
 Tissue paper wipes.
 Water-soluble lubricant for rectal temperature.
 Clean gloves for rectal temperature.
 Towel for axillary temperature.

1) Check pt. File and doctor order

2) prepare the equipment.
3) Clean the thermometer appropriately Soap, water, and alcohol
from bulb to glass in a
circular motion
4) Inspect the thermometer for any cracks,
chips,
or defects.

5) Shake down the mercury by holding the


thermometer between the thumb and
forefinger at the farthest from the bulb.
Stand away from solid objects, and sharply
flick the thermometer downward until the
mercury reading is below 35oC.

least 3 minutes, in the rectum for 3 to 5
minutes, and in the axilla for 10 minutes

20) Wipe the thermometer with soft tissue


from
the stem toward the bulb to read the
calibration accurately.

21) Hold the thermometer at eye level,


and rotate until the mercury column is
visible. The end of the column nearest
your hand indicates the client's body
temperature.
