LESSONS LEARNED (INTERNAL INCIDENT)
Well Flow Incident
WHAT HAPPENED? During pressure testing 3rd party pressure lines, observed well flowing from tubing side. Activated BOP to isolate annulus immediately and diverted flow from rig floor. Isolated tubing from gravel pack completion setting tool and secured well.
WHAT WENT WRONG? WHAT WERE THE CAUSES?
• Inadequate risk assessment in terms of monitoring downhole losses. • Surface testing lines were having multiple issues. • No stop work authority was exercised.
CORRECTIVE ACTIONS AND LESSONS:
• Review and update the Well Control Risk Action Plan for the inclusion of specialized jobs like Gravel Packs, Frac, ESPs. • For all the non-routine Completion Operations (like gravel packing operations), a separate plan for the well control situation needs to be added with all the detailed task steps. • Develop workflows for well control and monitoring during the completion testing phase both for perforated / non-perforated wells. • Issue standing instructions to all Well Site Leaders / service providers to ensure testing and proper function of the tools and equipment before mobilization at the rig site and present a “Fit for Job” or “Ready to Use” certificate to Well Site Leader. • Develop Well Control Incident Response team with Roles & Responsibilities and respective Training Need Analysis. Update Well Control Manual. • All the Well Site Leaders to be engaged for exhibiting proper behaviors during the well control challenging situations. • For high-risk critical non-routine activities, Driller’s instructions, needs to have second level review from Project Engineer. • Conduct a session with the Well Site Leader for gaps in the management of Well Control Risk.
