PEDIATRIC OVERVIEW

1. The child is prescribed fluconazole (Diflucan) 600 mg PO bid. The pharmacy supplies a
bottle labeled “fluconazole (Diflucan) 60 mg/mL.” How many tsp will the nurse
administer?

1 tsp 1 mL 600 mg 600

2. The child is prescribed acetylsalicylic acid (aspirin) 650 mg PO daily. The pharmacy
supplies acetylsalicylic acid (aspirin) 325 mg in scored tablets. How many tablets will
the nurse administer?

3. A child weighing 37 lb 5 oz is prescribed Rocephin.

Calculate the weight in kg (Round to the nearest whole number)-16
16 oz= 1 lb
5 oz/16 oz= 0.3125

37lb + 0.3125 = 37.3125 lb

37.3125/2.2= 16.9602273 kg

= 17 kg ( rounded to the whole number)

= 16.96 kg (rounded to the nearest hundredth)

3b. 7 lbs 9 oz=

9oz/16oz= 0.5625

7 + 0.5625 lb = 7.5625

7.5625 lb / 2.2

3.4375 kg= 3.44 kg (rounded to the nearest hundredth)

3c. 9 lbs 11 oz

11 oz/16 oz = 0.6875

9 +0.6875= 9.6875

9.6875 lb /2.2 = 4.40 = 4.4 kg

EXAMPLE
A doctor orders 200 mg of Rocephin to be taken by a 15 lb. 4oz infant every
8 hours. The medication label shows that 75-150 mg/kg per day is the
appropriate dosage range. Is this doctor's order within the desired range?
12lb. 3 oz= 5.54 kg (415.5 mg/day-831 mg/day) 8lb 8oz= 3.86 kg (289.5
mg/day-579 mg/day)

1. Convert 15 lb 4 oz to kg.
lb → kg ( ÷ by 2.2 )

15 lb 4/16 (0.25) 15.25 lb ÷ 2.2 = 6.93 kg

6.93 Nearest hundredth
7 nearest whole number

Determine safe dose range

2a. 6.93 kg x /day = 519.75 mg/day (Minimum Desired
75 mg/kg Dosage)

2b. 6.93 = 1,039.5 mg/day (Maximum Desired

kg x 150mg/kg/day Dosage)

24 hours in one day and the medication is ordered every 8

hours.
3

24 hrs / 8 hrs = 3 times per day doctor ordered medication

200 mg x 3 = 600 mg ordered per day

600 mg is within the desired range of 520-1,040 mg

Yes doctor has ordered a safe dose because it is within the desired range.

4. The child is prescribed omnicef (Cefdinir) 380 mg PO qid (4x day). The safe dose range
for omnicef (Cefdinir) is 15–30 mg/kg/day in 4 divided doses. The child weighs 105 lb.
Calculate the individual safe dose range for this child. Is the ordered dose safe? Round
the nearest tenth.

Step 1 Convert pounds to kg 105 lb./2.2 47.73kg

Step 2 Determine safe dose range

a. Low dose 47.73 kg x 15 mg/kg/day = 715.95 mg/day in 4 divided doses

b. High dose 47.73 kg x 30 mg/kg/day = 1431.9 mg/day in 4 divided doses

Ordered dose is 380 mg po QID= dose ordered is too high (380 mg x 4= 1520 mg)

5. The physician orders duramorph (Morphine Sulfate) 2 mg IV q4h prn pain. The safe
dose range is 0.1–0.2 mg/kg/dose. The child weighs 43 lb (19.55 kg). What is the
individual safe dose range for this child? Is the ordered dose safe?

mg 0.1 mg 1 kg 43lb 4.3

Low dose = → → = =1.9 6 mg per individual dose .
dose kg/dose 2.2 lb 1 2.2

mg 0.2 mg 1 kg 43lb 8.6

High dose = → → = =3.91 mg per individual dose .
dose kg/dose 2.2 lb 1 2.2

T h e safe dose range is 1.9 6 – 3.91

( 2−4 mg per dose rounded ¿nearest w h ole number )
T h e ordered dose is safe .

1. Convert weight from pounds to kilograms 43/ 2.2= 19.55 kg

2. Calculate low dose - 19.55 kg x 0.1mg/kg/dose (1.955 mg) = 1.96 mg/dose
3. Calculate high dose= 19.55 kg x 0.2 mg/kg/dose = 3.91 mg/dose
4. Range is 1.96 mg/dose - 3.91 mg/dose
5. Ordered dose is 2 mg, therefore it is safe
4

6. Order: amoxicillin (Augmentin) 175 mg po q 8hr (3 doses)

Child: weight 30 lb (13.64)
Supply: 125 mg/ 5mL
Literature 40 mg/kg/day in doses q 8 hr

a. Is this a safe dose range for the child?

b. How many mL will the child receive? (Using whatever method you use to
calculate)

STEPS:
1. Convert pounds to kilograms
2. Determine safe dose range
3. Is this a safe dose range?
4. Calculate dose to be given.

1. 30 lb./2.2= 13.6363636 kg = 13.64 kg

2. Calculate maximum dose range

13.64 kg x 40 mg/kg/day = 545.6 mg/day

175 mg x 3 times a day (q 8 hr) = 525 mg in a 24/hr (daily dose)

Formula: 175 mg x 5 mL
125 mg = 7 mL formula method

DA: 5 mL 175mg
125 mg 1 = 7 mL dimensional analysis

Proportion as two fractions

5 mL x mL
125 mg 175 mg

175 mg x 5 = 125x

875 mg = 125x
125 125

7 mL= x

10. Order: Dilantin po 60 mg/day

Supply 30 mg/5 mL
Literature states: 4-8 mg/kg/day. Max dose 300 mg/day
5

Weight 12 lb 8 oz = 8 oz/16 = 0.5 12.5 lb /2.2 = 5.68 kg

4-8 mg/kg/day. Max dose 300 mg/day

5.68 kg x 4 mg/kg/day = 22.72 mg/day

5.68 kg x 8 mg/kg/day = 45.44 mg/day
How many mL will you give the patient - NONE, call M.D. dose not safe (too high)
