VB-MAPP

Assessment Kit
How to Setup:
Materials Needed:
• two or three 3-inch binders
• extra wide dividers
• page protectors
• laminating sheets
• velcro
• clear packing tape
• ziploc bags

The VB-MAPP Assessment Kit is designed to fit within three 3-inch binders. There
are a few options for how to setup task cards depending on how much laminating
you would like to do. Extra wide dividers make it easier to locate different milestone
resources.

Setting up binder system:


After printing the entire resource, use the label pages to label the
dividers. Put the small label on the tab and one of the large labels on the
front. The other large label can be used to label a ziploc bag with the

Setting up task cards:


Most of the milestones include task
cards as the assessment resource.
There are a few options on how to
set these up. You can laminate and
cut out. Store in a ziploc bag and
velcro to the back of the divider.
You can also put the full pages in
top loading sheets.
Setting up matching activities:
Some milestones include matching
activities. For these activities
laminate the pages and pieces.
Pages could also be put in top
loading sheets. Hole punch the
pages and store behind the divider
for each milestone.

Organize data sheets.

I recommend storing masters of the data sheets in each resource binder.


Copy data sheets for individual students as needed.
How to Use:
Use with the Verbal Behavior Milestones Assessment and Placement Program. All
of these materials are color coordinated and set up in a system that is extremely
easy to utilize. This resource is designed to have all of the materials at your finger
tips when you need them.
• The resources are organized into 2 main sections: Levels 1 and 2 and Level 3.
• All of the materials are color coded so you can easily locate each task. See
Color Coded Guide for more information.
• Some resources can be utilized for multiple milestones. Each task is labeled
with all tasks included.
• When implementing the assessment, you can easily locate the task by name or
task number and assess the skill with your student.

For each level there is an Observation Data Sheet, Testing Data Sheet, and
Caregiver Questionnaire.

Observation Data Sheet


The observation data sheet organizes all of the assessment
milestones for each level that requires observation. The milestone
information is listed. Conduct one or several observations to note the
occurrence of the various behaviors. Add information to the tracking
chart when completed.

Testing Data Sheet

The testing data sheet organizes all of the assessment milestones


for each level that requires formal testing. The milestone information
is listed. Conduct several sessions using the assessment material
binders. Add information to the tracking chart when completed.

Caregiver Questionnaire
The caregiver questionnaire organizes all of the assessment
milestones for each level that requires observation. All milestone
information is set up in easy to understand questions and all
questions list the relevant milestone. Use the questionnaire
information to support observation data or gain information that may
not be able to be observed within the current setting.
